Responsive – this means we looked for evidence that the provider met people’s needs.At our last assessment we rated this key question Good. At this assessment the rating has changed to Requires Improvement. This meant people’s needs were not always met. The service was in breach of legal regulation in relation to person centred care.Care plans were not sufficiently detailed or person centred. People’s likes and dislikes and life histories were not captured within care plans and risk assessments.

Person-centred Care

While some of the people we spoke to expressed that they were generally happy with their care, our assessment found care did not always meet the expected standards. The provider did not make sure people were at the centre of their care and treatment choices and they did not work in partnership with people, to decide how to respond to any relevant changes in people’s needs. Care plans were not person centred or person specific and contained very little information about people’s lives, experiences and likes and dislikes. When incidents occurred, or people’s needs changed there was no update to the care plan. One relative told us their loved one did not have a care plan for them to review within their home, despite them wanting this. There was no information for staff to use to build relationships with people, or even for conversation starters. One relative told us that care calls could not be scheduled in the evening to meet their loved ones needs.

Equity in experiences and outcomes

Staff and leaders did not always actively listen to information about people who are most likely to experience inequality in experience or outcomes. This meant people’s care was not always tailored in response to this. People with protected characteristics were not considered and adjustments implemented within their care plans or support to inform staff how best to support them. People’s human rights were not always upheld as the provider did not consider people’s capacity and relied on relatives and next of kin’s to make decisions on people’s behalf.
