Assessing needs
We scored the service as 3. The evidence showed a good standard. The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.
The service now had an effective system to identify and support people with additional needs. Leaders had addressed the issue we previously identified regarding supporting people with additional needs. Across the whole franchise, the service had updated its pre-scan questionnaire within the online pre-booking system to identify anyone who may require additional assistance in advance of their appointment, allowing the service to ensure support was available. We reviewed a mock-up of the system, which asked patients about communication, sensory, mobility and access needs before their appointment. The system was not yet live at the time of the assessment; the leaders who would confirm the go live date were on leave and the service agreed to inform us once it was launched.
Patient care records were generally of good quality and complete. We reviewed 5 patient care records which included the preadmission terms, the conditions assessment and the scanning reports. A condition assessment is a clinical evaluation of a patient's health status, symptoms, medical history, and presenting concerns. It enables clinicians to identify actual or potential health risks, determine the required course of action, and ensure that patients received care that meets their individual needs. The preadmission assessment included details of any allergies, condition, previous pregnancy information (including number of miscarriages), along with details of the service planned and cost. All assessments were signed and dated by the patient attending. Scan reports included pregnancy dates, details of sonographer and chaperone, confirmed consent and then the details of the scan. Where anomalies were identified, there was clear written and verbal advice on what the patient should do. An anomaly is any result or finding outside the normal expected range, which may indicate a health concern requiring further investigation or monitoring. It is essential that patients are informed of such findings so they understand their significance and can take action, including seeking further medical advice or treatment where necessary.
