The service protected patients from avoidable harm and maintained safe systems of care. Staff understood how to report incidents, and leaders investigated and shared learning to improve practice. Risks were assessed and managed using recognised tools and processes, including consistent safety checks across surgical pathways. Staffing levels were appropriate, and staff had the skills and training needed to provide safe care. Infection prevention and control, medicines management and safeguarding arrangements were effective. Environmental risks were identified and monitored, although some storage pressures in theatre department remained and required ongoing review.
