Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. For example, they had created educational videos to put on social media on health topics such as diabetes, flu, weight management, menopause and vitamin D.

The practice had a process for identifying patients with specific care needs and staff members took the lead for these groups such as cancer, end of life and learning disabilities. They undertook searches on a monthly basis to keep these lists up to date and to ensure targets such as health checks were met.
