Swanholme Court is a care home that provides support for older people and people living with dementia. The service is registered to support 27 people, at the time of the inspection, 26 people were using the service.
Systems were in place to support people’s safety, including safeguarding, risk management, infection prevention and medicines management. However, these were not always applied consistently in practice. Recording and oversight did not always fully demonstrate how learning was identified and used to improve practice, particularly in relation to incidents and risk management. Care planning did not always provide sufficiently detailed guidance for staff, especially where people experienced distress, which could lead to some variation in how support was delivered.
Staff were generally well trained and supported in their roles, and staffing levels met people’s needs. Some staff described periods of pressure and reduced morale during a time of change within the service. A new registered manager had started at the time of the inspection. While they were in the early stages of the role, there was an expectation they would provide greater stability and direction following a period of change.
People received care from staff who were kind, caring and respectful. Positive relationships were evident and staff supported people to maintain their independence, make choices and take part in activities within the home and the local community. Staff demonstrated a good understanding of people’s individual needs and, in most cases, delivered care in a person-centred way.
People were supported to access healthcare services, and their health needs were monitored. Staff worked with external professionals and followed guidance where required.
Leaders were reflective and had identified areas requiring improvement. There was a clear commitment to strengthening systems and ensuring consistent, high‑quality care for people.
