Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. Care plans were in place and had been reviewed regularly. Relatives told us they were invited to care plan reviews and involved in the initial assessment process.

Care provision, Integration and continuity

Score: 3

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. Staff had a good working relationship with local health professionals and made sure that people received joined up care. One health professional said, “I have a good relationship with the staff, they are fab.”

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs.

People received information in a way that met their individual needs. Care plans included a communication section which included details of health conditions which may affect people’s ability to communicate and guidance for staff to follow.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result.

We received positive feedback from relatives about being involved in care plan reviews and being kept informed of people’s changing needs.

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it.

People were supported to attend appointments such as hospital appointments.

The need for referrals to health care professionals were identified and made in a timely manner.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. Staff worked pro-actively with local healthcare professionals to ensure people could access services when they needed to. One health professional told us, “The staff here make their own referrals to the appropriate teams where they can.”

Planning for the future

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life.

Care plans included detailed information about people’s wishes at the end of their life, including details of any resuscitation and hospital admission decisions that had been made.
