Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

Relatives confirmed people’s needs were assessed and felt involved with this process.

Care plans and risk assessments were developed by observing people, based on these assessments. People’s care was regularly reviewed to ensure it was up to date.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them.

The leadership team told us they had a system in place which used evidence-based tools to assess people’s needs. A variety of assessment tools were available if needed to ensure people’s needs were identified and met. Staff were aware of these outcomes to ensure they delivered the correct care that people needed.

How staff, teams and services work together

The provider worked well across teams and services to support people.

Staff worked well together, this included sharing information about people. There was a system in place to ensure the staff team worked together and alongside other professionals to deliver care to people.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control.

Staff were aware of people’s health needs and the support they may need. People’s health needs were assessed, and plans were in place to monitor these conditions, where needed. People had access to health professional when needed including the GP.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it.

Leaders shared with us examples of how improvements had been made to people’s well-being and life since moving to the service.

There were systems in place to ensure people’s needs were identified, assessed and reviewed to ensure positive outcomes were achieved for people. It was ensured people led on their care, with support from professionals and were involved with any changes. Staff confirmed they were aware of people’s assessed needs and how they delivered support in line with this.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

Capacity assessments and best interest decisions were in place when needed to ensure the Mental Capacity Act was applied. Staff had received training in this area and were aware of the processes to follow.
