Well-led

Good

13 October 2026

Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant the service was consistently managed and well-led. Leaders and the culture they created promoted high-quality, person-centred care.

The provider was previously in breach of the legal regulation in relation to good governance. Improvements were found at this assessment, and the provider was no longer in breach of this regulation.

This service scored 75 (out of 100) for this area. Find out what we look at when we assess this area and How we calculate these scores.

Shared direction and culture

Score: 3

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities. The provider had taken steps to make improvements in relation to this since our last assessment. Whilst we continued to find a positive culture, and the drive to provide a kind and caring service by all those who worked for the service, the steps the provider had taken in relation to governance had strengthened this further. For example, robust records were now being kept in relation to incidents that showed lessons learnt and how these had been implemented and shared with staff. Furthermore, we saw the provider worked collaboratively with other professionals in an open and transparent manner.

Capable, compassionate and inclusive leaders

Score: 3

The provider had inclusive leaders at all levels who understood the context in which they delivered care, treatment and support and embodied the culture and values of their workforce and organisation. Leaders had the skills, knowledge, experience and credibility to lead effectively. They did so with integrity, openness and honesty. Whilst the registered manager and senior leadership team demonstrated empathy, kindness, and care at our last assessment, which continued to be demonstrated at this one, actions had been taken to strengthen their knowledge in relation to compliance with legislation and regulation. For example, the registered manager had completed several training courses since our last assessment and demonstrated, through records, a better understanding of their registered role. They had also commenced completing their own audits of the service to strengthen oversight. Professionals who provided us with feedback consistently told us the management team worked with them in an open, transparent, and collaborative manner which was respectful, engaging, and for the benefit of people who used the service. For example, one said, “My experience of Riseup has been positive. I have consistently found the service to be person centred, responsive, and committed to achieving positive outcomes for the people they support. The management team and care staff demonstrate a good understanding of individuals' needs and work collaboratively with professionals and families to ensure care remains appropriate.”

Freedom to speak up

Score: 3

The provider continued to foster a positive culture where people felt they could speak up and their voice would be heard. People, and staff, told us this. One person who used the service said, “I can speak to [office staff member] and it always gets done” whilst another told us, “[Registered manager] is very respectful and responsive.” A staff member said, “In my experience [the Registered Manager] is kind, compassionate and always willing to help. They are approachable and makes themselves available to support the team, including providing advice and guidance when needed. I have found their commitment to the service evident in the way they support both staff and clients. They demonstrate genuine care and dedication to the service.” We saw that the provider had a policy in place to support people in speaking up.

Workforce equality, diversity and inclusion

Score: 3

The provider continued to value diversity in their workforce. They worked towards an inclusive and fair culture by improving equality and equity for people who worked for them. Staff spoke positively about how they were treated at work, telling us they felt valued, listened to, and supported. One staff member said, “My experience working at Riseup Healthcare has been excellent - I have worked in many different organisations during my career and can honestly say that I have never felt so valued and part of ‘the team’ as I do here. Every member of staff is supportive, committed and compassionate.”

Governance, management and sustainability

Score: 3

The provider had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate. Following our last assessment in June 2026 where shortfalls were identified, the provider had taken robust and proactive action to improve the governance system in place. For example, they had employed further staff to help manage this aspect of the service, employed the skills of a consultant and introduced a wide-reaching, detailed, and meaningful governance system. Whilst the new governance system needs to continue to be embedded and further improvements made, it had been successful at rectifying the shortfalls found at the previous assessment. Furthermore, with continued work and embedding, the system has the potential to ensure the service is robustly monitored and continually improved.

Partnerships and communities

Score: 3

The provider continued to understand their duty to collaborate and work in partnership, so services worked seamlessly for people. They shared information and learning with partners and collaborated for improvement. The many professionals who provided us with feedback as part of this assessment, told us this. One said, “Based on my professional experience, Riseup Healthcare are responsive, flexible and person-centred in their approach. They work well in partnership with statutory services and demonstrate a genuine commitment to supporting people to remain safe, well and as independent as possible.”

Learning, improvement and innovation

Score: 3

The provider focused on continuous learning, innovation and improvement across the organisation and local system. They encouraged creative ways of delivering equality of experience, outcome and quality of life for people. They actively contributed to safe, effective practice. We have been assured that the provider has learnt from the shortfalls and subsequent enforcement action taken following the last assessment in June 2026. They have demonstrated robust action in response and that they are committed to ensuring the people who use their service receive consistently caring, safe, and effective care. Whilst the people who use the service, and their relatives, have consistently been happy with the service they have received, staff have provided us with feedback to demonstrate improvements have been made that have had positive impact. For example, one staff member said, “I’ve seen changes. Care plans are more detailed, MAR [Medication Administration Record] charts are clearer, supervisions and spot checks are more regular, and training is more up to date.It helps me feel more confident and service users get better care.”
