Governance, management and sustainability
The practice had a governance system that included policies and procedures, which were accessible to staff and were reviewed on a regular basis.
A significant amount of work had been undertaken by the covering Practice Manager in the period leading up to the inspection to provide evidence and address gaps they had identified, including in relation to staff training and recruitment records.
Some processes, such as the ongoing management of Legionella and fire safety checks, had also recently been reintroduced. However, systems and overall oversight and governance had not been sufficiently effective in identifying gaps or ensuring that processes were consistently embedded and maintained.
Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations.
The group had relevant policies and protocols were in place for the use of closed-circuit television (CCTV).
There were effective processes for identifying and investigating incidents and accidents, and for receiving and acting on safety alerts.
The practice had systems and processes for learning, quality assurance and continuous improvement. Audits had identified areas for improvement, and this was evident in the records we reviewed on the day. The practice had taken action in response to the findings, demonstrating that audits were being used effectively to identify and drive improvements.
Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement.
Staff had clear responsibilities, and systems of accountability to support good governance.
Staff described a range of communication channels, including practice meetings and online chat groups. Some staff said they were kept informed of changes and future plans, while others felt there could be more opportunities for staff to contribute to future planning.
The practice had taken steps to improve environmental sustainability. For example, staff told us they switched off lights when not required and recycled paper and cardboard.
