At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good.

This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People confirmed their needs were assessed. Care plans and risk assessments were developed alongside people, based on these assessments. People’s care was reviewed to ensure it was up to date.

Delivering evidence-based care and treatment

The provider could not always evidence that people’s care and treatment was based on the best available evidence.

The records we viewed did not show evidence-based tools were always used to assess and deliver people’s care. Improvements were needed to care plans and risk assessments to include this information to show how the care and treatment delivered was evidence based. However, staff were able to describe how they delivered care to people and told us they understood people’s needs. People raised no concerns with the delivery of their care.

How staff, teams and services work together

The provider worked well across teams and services to support people.

People told us how staff worked well together. One person told us they had a care plan that the carers wrote in once a day.

Staff worked well together, this included sharing information about people through daily notes. There was a system in place to ensure the staff team worked together, this included when people needed support from other health professionals.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control.

People confirmed their health needs were responded to.One person explained that if needed the carers would arrange an appointment for them with the GP. Carers would also assist them with collecting medicines from the pharmacy if family were not available. Staff were aware of people’s health needs and the support they needed.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it.

There were systems in place to ensure people’s needs were identified, assessed and reviewed to ensure positive outcomes were achieved for people. Staff confirmed they were aware of people’s assessed needs and how they delivered support in line with this.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

The provider was not currently supporting anyone who was not able to consent to their care. However, staff were able to explain the action they would take if people’s ability to consent to their care changed. Records confirmed staff had received training in this area.
