During a routine inspection

Cotman House provides accommodation and personal care for up to 41 older people who require 24 hour support and care. Some people are living with dementia.

There were 41 people living in the service when we inspected on 20 October 2014. This was an unannounced inspection.

A registered manager was in post. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons.’ Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

Staff were provided with the information that they needed to safeguard the people who used the service from abuse. Staff understood the various types of abuse and knew who to report any concerns to.

There were appropriate arrangements in place to ensure people’s medicines were obtained, stored and administered safely.

There were sufficient numbers of staff who were trained and supported to meet the needs of the people who used the service. Staff were available when people needed care and support.

Staff had good relationships with people who used the service and were attentive to their needs. Staff respected people’s privacy and dignity and interacted with people in a caring, respectful and professional manner.

Staff in the service were trained and knowledgeable about the Mental Capacity Act (MCA) 2005. The MCA sets out what must be done to make sure that the human rights of people who may lack mental capacity to make decisions are protected, including when balancing autonomy and protection in relation to consent or refusal of care or treatment.

People, or their representatives, were involved in making decisions about their care and support. People’s care plans had been tailored to the individual and contained information about how they communicated and their ability to make decisions. The service was up to date with recent changes to the law regarding the Deprivation of Liberty Safeguards and at the time of the inspection they were working with the local authority to make sure people’s legal rights were protected.

People were supported to see, when needed, health and social care professionals to make sure they received appropriate care and treatment.

People’s nutritional needs were being assessed and met. Where issues were identified, for example, where a person was losing weight, appropriate referrals were made to other professionals. The service took action to ensure that people’s dietary needs were identified and met.

People knew how to make a complaint if they were not happy with the service they were provided with. People’s concerns and complaints were listened to, addressed in a timely manner and used to improve the service.

Staff understood their roles and responsibilities in providing safe and good quality care to the people who used the service. The service identified shortfalls in the service provision and took actions to address them. As a result the quality of the service continued to improve.

During a routine inspection

We spoke with seven people who used the service who told us that they were happy living in the service. One person said, "We are all very well looked after." Another person said, "I have no problems at all." Another said, "I am looked after, always something to do, I have made some very good friends."

We spoke with two people's visitors who told us that they were satisfied that their relative/friend was being cared for appropriately. One visitor said, "It is excellent, staff are extremely good, we are very happy with what they provide." Another visitor said that their friend was, "Coming on a treat," since they had moved into the service.

We looked at the care records of five people who used the service and found that they experienced care, treatment and support that met their needs and protected their rights.

We found that people were protected from the risks of unsafe or inappropriate care and treatment because accurate and appropriate records were maintained.

We looked around the service and saw records relating to infection control and the maintenance and servicing of equipment, such as lifting equipment. We found that people were cared for in a clean, hygienic environment and they were protected from unsafe or unsuitable equipment.

We looked at the personnel records of three staff members and found that the appropriate checks were made on staff before they started working in the service to ensure that they were able to work with vulnerable people.

During a routine inspection

We spoke with seven people who used the service. People told us that they were consulted about the care they were provided with and that the staff listened and acted on what they said. One person said, 'I am always asked about my care and they (staff) check what I want.' Another person said, 'They (staff) always ask what I like and don't like.'

People who used the service told us that they were happy with the care they were provided with. One person said, "I am very happy overall." Another person said, "I get looked after well." Another said, "Things here are very good. They (staff) are very caring and the food is very good." Another said, 'If I need help from the staff it is never too much trouble.'

People spoken with were complimentary about the approach of the staff who supported them and told us that the staff treated them with respect. One person said, "They (staff) are always smiling and professional." Another person said, 'The staff are really good.' Another said, 'They (staff) are very caring.'

We tracked the care records of four people who used the service which showed that people experienced care, treatment and support that met their needs and protected their rights.

During a routine inspection

We spoke with five people who used the service who told us that the staff treated them with respect and were attentive to their needs. They told us that they were consulted about the care and support that they were provided with and that the staff listened and acted upon what they said.

People told us that they were provided with enough to eat and drink and that they were always offered choices of meals. They said that the food was of good quality.

People who were spoken with said that there were activities provided that they could choose to participate in.

People were complimentary about the environment that they lived in.
