Accommodation for persons who require nursing or personal care
Mr Adam Knights is responsible for these services.
Condition of this registration relating to carrying out this regulated activity
The Registered Person must not admit any new service user without the prior written agreement of the Care Quality Commission. This includes any respite or emergency admissions of service users, any readmissions of previous service users, or where service users return to the location following admission to hospital.
The Registered Person must urgently and without delay record and monitor all occurrences of chemical and physical restraint over the last three months from the date of this Notice. A themes and trends analysis of all occurrences of chemical and physical restraint used by staff must be carried out. The analysis must: -
a. Identify incidents of a safeguarding nature which may require reporting.
b. Be used to inform an urgent review and updating of care plans and risk assessments where restraint is in place.
c. Identify that correct terminology is used to record restraint taking place and where not, amend.
d. Ensure that appropriate debriefs occur and are documented following the use of any restraint.
The Registered Person must review this position every 28 days to ensure that any changes have been appropriately documented and new service users have been included.
The Registered Person must urgently and without delay ensure behaviour management risk assessments, care plans and mental capacity assessments for all service users are reflective of professional advice from a Positive Behaviour Support specialist. The Registered Person must review these documents every 28 days to ensure that any changes have been appropriately documented and new service users have been included.
The Registered Person must urgently and without delay ensure skin integrity management and smoking risk assessments and care plans for all service users are reviewed. The Registered Person must review this position every 28 days to ensure that any changes have been appropriately documented and new service users have been included.
By Friday 15 September 2023, all staff including agency staff providing personal care to service users must have suitable training and competency checks for managing service user’s risk behaviours safely. This must include training in restrictive practices. Training must be delivered by a person with the necessary skills and qualifications. The Registered Person must review this position every 28 days to ensure that all staff are trained and competent upon recruitment.
The Registered Provider must develop an action plan and update in relation to conditions 1 to 5. This must include information of any actions taken, improvements and any proposed action in regards of the above conditions. This must be sent to the CQC by 22 September 2023.
By 6 October 2023, the Registered Person will ensure that there is suitably qualified clinical leadership, physically present within Drovers Call, a minimum of 40 hours per week, worked over a minimum of five days.
By 6 October 2023, the registered provider must ensure that audit and review of the delivery of clinical care at Drovers Call is carried out for all service users with long term medical conditions including but not limited to diabetes and epilepsy, taking into account NICE guidelines when caring and supporting service users with long term medical conditions. The review must:
a. Identify immediate risks associated with service users care needs and inform an urgent review and updating of care plans and risk assessments where long term medical conditions are identified.
b. Ensure escalation to appropriate medical professionals takes place where necessary.
c. Identify where care plans, risk assessments and health monitoring records are missing for medical conditions and put these in place immediately.
d. Ensure service users have personalised care plans and risks assessments in place.
By 4:00pm on 6 October 2023 the registered provider must provide a report to the Commission setting out a summary of the audit and reviews confirming compliance with the above conditions. Thereafter, on the first day of each month the registered provider must submit a monthly report to the Commission about the outcome of the review and audits. This report must include actions taken as a result of the reviews and audits to ensure any risks identified have been assessed and mitigated appropriately. This report must specify when the actions have been or will be completed and highlight who is responsible to ensure that the risks are mitigated.
