Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, well-being and communication needs with them. Initial assessments were thorough and included input from families and professionals. Care plans were detailed and person-centred, reflecting people’s health conditions, preferences, and routines. Staff used this information to deliver consistent and safe care.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. The provider used nationally recognised tools to help inform their risk assessment process. Staff completed visit logs which we saw captured support which had been delivered in line with people’s choices.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. People received support from consistent teams, and communication between staff and families was strong. Staff described working closely with relatives to ensure care was tailored and responsive. A relative said, “They [staff] always turn up, stay, and are kind and friendly. Communication is good.”

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Care plans included information about diet, exercise, and emotional well-being. Staff monitored health conditions and responded to changes. People were encouraged to engage in social activities and maintain routines that supported their wellbeing.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. Care plans were updated regularly, and staff used feedback from people and families to improve care.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Care plans included documented consent for information sharing and care delivery. Staff understood the importance of gaining consent and described how they involved people in decisions about their care. A person said, “They ask for consent and know me well.”
