The clinic had no serious incidents reported. While staff worked collaboratively and confidentiality arrangements were effective, the inspection identified significant governance failures. Key concerns included poor oversight of complaints, ineffective audits, inadequate risk management, unresolved actions on the risk register, insufficient patient follow-up, weak document control with many overdue policies and SOPs, poor infection prevention and control procedures, and inadequate medicines, stock and equipment management. Overall, the provider could not demonstrate effective governance, assurance or monitoring processes to ensure risks, quality and patient safety were consistently managed, resulting in multiple breaches of good governance.
