During an inspection looking at part of the service

We carried out this announced inspection on 1 July 2021 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered provider was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a Care Quality Commission, (CQC), inspector who was supported by a specialist dental adviser.

To get to the heart of patients’ experiences of care and treatment, we always ask the following five questions:

• Is it safe?

• Is it effective?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

We found this practice was providing safe care in accordance with the relevant regulations.

We found this practice was providing effective care in accordance with the relevant regulations.

Are services well-led?

We found this practice was providing well-led care in accordance with the relevant regulations.

Background

Boston Dental Centre is in the town of Boston, Lincolnshire and provides NHS dental care and treatment for adults and children.

There is level access to the practice for people who use wheelchairs and those with pushchairs. Car parking spaces, including dedicated parking for people with disabilities, are available near the practice.

The dental team includes 8 dentists, 10 dental nurses, 6 of whom were trainees, one dental hygienist, a practice manager and two administrators. The practice has 9 treatment rooms.

The practice is owned by a company and as a condition of registration must have a person registered with the CQC as the registered manager. Registered managers have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated regulations about how the practice is run. The registered manager at Boston Dental Centre is the practice manager.

During the inspection we spoke with three dentists, five dental nurses, one administrator and the practice manager. We looked at practice policies and procedures and other records about how the service is managed.

The practice is open:

Monday, Wednesday and Friday 8 am – 5 pm

Tuesday 8 am – 6 pm

Thursday 8 am – 7 pm

Saturday 9 am – 4 pm

Sunday Closed

Our key findings were:

  • The practice appeared to be visibly clean and well-maintained.
  • The provider had infection control procedures which reflected published guidance.
  • Staff knew how to deal with emergencies. Appropriate medicines and life-saving equipment were available.
  • The provider had systems to help them manage risk to patients and staff.
  • The provider had safeguarding processes and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • The provider had staff recruitment procedures which reflected current legislation.
  • The clinical staff provided patients’ care and treatment in line with current guidelines.
  • Staff treated patients with dignity and respect and took care to protect their privacy and personal information.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • The appointment system took account of patients’ needs.
  • The provider had effective leadership and a culture of continuous improvement.
  • Staff felt involved and supported and worked as a team.
  • The provider asked staff and patients for feedback about the services they provided.
  • The provider dealt with complaints positively and efficiently.
  • The provider had information governance arrangements.

There were areas where the provider could make improvements. They should:

Implement a system to ensure patient referrals to other dental or health care professionals are centrally monitored to ensure they are received in a timely manner and not lost.

During a routine inspection

Boston Dental Centre had a beauty therapy room at its premises. The procedures undertaken in the therapy room were not required to be regulated by the Care Quality Commission. The evidence in this report is for the dental practice.

During our inspection we spoke with six members of staff and seven patients who used the service as well as the registered manager/ practice manager.

The patients we spoke with were complimentary about the way staff interacted with them and the treatment they received. One patient told us, 'They told me step by step what I needed to have done and gave me choices.' Patients also told us they were respected by staff.

The practice had systems in place to reduce the risk of infection. Patients told us dentists and nurses always washed their hands and used masks and gloves whilst they were receiving treatment.

Patients told us they were confident in the staffs' abilities and said they thought they were competent. One patient told us, 'They seem to be very knowledgeable about things. 'Staff felt fully supported by the registered manager. However, we found dentists had not received regular documented appraisals.

Patients were asked for their views and comments about the service. There were systems in place to make sure risks to patients were identified and managed effectively.
