This assessment was carried out from 27 to 29 April 2026. This was a planned comprehensive assessment following registration and was completed under the single assessment framework.
Core Case Management Ltd is a domiciliary care service registered to provide personal care to up to 5 adults living with a physical disability, sensory impairment and/or mental health. At the time of this assessment, 5 people with complex physical and neurological conditions, including acquired brain injury, spinal injury and poly-trauma, were receiving support. This was the first assessment for the service.
The provider delivered safe, effective,compassionate and person-centred care that was tailored to people’s individual needs, preferences and aspirations.
The provider had strong systems in place to assess and manage risks, including enabling positive risk-taking, which supported people to maintain independence and live meaningful lives. Care planning was highly detailed, informed by multi-disciplinary professionals, and supported coordinated and responsive care.
The provider protected people from abuse and avoidable harm. Staff understood safeguarding procedures and were confident to raise and report concerns. There were robust systems in place for medicines management, infection prevention and control, staffing, training and competency assessment.
The provider ensured care and support achieved consistently good outcomes. People were supported to maintain relationships, access healthcare, pursue interests and take part in community activities. The provider worked effectively with healthcare professionals to ensure continuity of care.
The provider treated people with kindness, dignity and respect. Staff knew people well and promoted independence, choice and control in all aspects of daily life.
The service was well-led. Leaders were visible, knowledgeable and supportive, promoting an open and positive culture. Governance systems provided clear oversight of quality, safety and outcomes, and supported continuous learning and improvement.
However, the provider recognised areas for ongoing development, including embedding refresher training and strengthening the consistency of formal consent recording processes.
No breaches of regulation were identified at this assessment.
