Our current view of the service

Date of assessment 30 September 2025. Tall Trees is a residential care home providing personal and nursing care to up to 44 people. At the time of the inspection there were 38 people living there. Tall Trees accommodates 34 people across 2 separate wings. The service specialises in providing care to people living with dementia and mental health support needs.

Staff interactions were kind and caring they treated people with respect and dignity. There were systems in place to support people and staff which ensured equality, diversity and human rights. There were processes in place for people to speak up if they had any concerns.

The environment was maintained to keep people safe, however areas of the home were showing signs of wear and tear and in need of attention. We also signposted the provider to consider the design of the environment so that it meets the needs of people who may live with dementia or a dementia related condition to help them keep orientated, informed and to maintain their independence.

The home was mostly clean and hygienic; however, we did see some evidence of poor housekeeping on our first assessment visit which we shared with the management team. Personal protective equipment was in place and available for staff as and when they required it.

Overall, individual risks to people were identified and manged, they were supported by staff to live healthier lives and to be as independent as possible, reducing their future needs for support and care. However, this was not always consistent Healthcare professionals told us of anomalies with continuity of care, communication and safeguarding's not always being reported in a timely manner which meant delays to people receiving appropriate care and treatment.

We received mixed feedback from staff regarding the running of the service, with many staff members not feeling listened to and supported in their role by the approach of the management team. The registered manager was receptive to our feedback of the shortfalls and inconsistencies identified at the inspection and told us of the actions they were taking to address the concerns. This included implementing improvements to the culture, so staff felt able to speak up and raise concerns. We were encouraged by the actions taken but these measures need to be sustained and embedded into the service to demonstrate continual development.

People's experience of the service

During our inspection we observed how people were supported to ensure their needs were met. We spoke with them throughout the day, and an expert by experience spoke with people and to gain their relatives views. Most of the feedback was positive and we saw that staff knew people well and anticipated their needs. While people expressed general satisfaction with their environment, our inspection found the décor of the building did not meet expected standards.

One relative told us, “I had no experience of any care homes before my [family member] came to Tall Trees, but I would definitely recommend it.” However, another relative shared the inconsistencies they had experienced due to staffing arrangements, “The issues the home has at present I hope is a blip as I can only refer back to when my [family member] first entered [quite unwell] and the staff worked incredibly hard and the level of care was excellent, at that time. I will forever be grateful for this. It saddens me that for the last year it has been tough on the staff and a great concern for the relatives however, in recent weeks there are signs that things are starting to improve.” We shared feedback with the registered manager who acknowledged the feedback, of the strengths and improvements required at the service.
