Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. This is the first assessment for this newly registered service. This key question has been rated good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider did not always make sure people’s care and treatment were effective because they did not always check and discuss people’s health, care, wellbeing and communication needs with them. People’s needs were assessed when they started using the service, but some people and relatives said they had not been consistently reviewed since. One person told us there had, “Not been any reviews”. The registered manager had identified this as an issue, and a plan was in place to involve people and relatives in regular reviews of their care.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. This included staff providing effective support with eating and drinking, with people telling us they received the meals they wanted.

How staff, teams and services work together

Score: 3

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff were quickly updated on any changes to people’s support needs and choices, which helped them to provide effective care. One member of staff said, “The (care records) app is good, has all the information we need. We get emails with updates.”

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Staff understood people’s care needs well, and where required people were supported to access healthcare partners to protect and promote their health and wellbeing.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. Care records contained evidence of the effective monitoring of people’s outcomes, with staff able to tell us how they worked to improve these.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. People’s consent to their care was sought, and where Lasting Powers of Attorney were in place these were clearly recorded.
