During an inspection looking at part of the service

About the service

Elite Home Care Solutions (UK) is a domiciliary care service that provides personal care and support to people living in their own homes. The service supported 53 people at the time of the inspection.

Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do we also consider any wider social care provided.

People's experience of using this service and what we found

People and their relatives were positive about the support they received from staff. People told us they felt safe when staff visited. They were confident that staff provided them with personalised support which was in accordance with their wishes and preferences. People told us if they had any concerns, these would be dealt with promptly. Sufficient numbers of staff were employed to deliver the care and support people required. Where people were supported with medicines, they received support from staff who had received training around this and medicines were managed safely.

Infection prevention measures had been established within the service. Staff had a good understanding of these procedures and people confirmed staff were wearing protective equipment when visiting them in their homes.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

The manager acted on concerns to ensure people received care which was safe and responsive to their needs. Staff were trained in safeguarding people and protecting them from harm. Any concerns or accidents were reported and acted on.

The manager had developed quality assurance systems to monitor the overall quality of the service provided to people. These systems had led to improvements in the quality of the service being provided to people.

Rating at last inspection (and update)

The last rating for this service was requires improvement (published 14 March 2020) and a breach of regulation was found. The provider completed an action plan after the last inspection to show what they would do and by when to improve. At this inspection we found improvements had been made and the provider was no longer in breach of regulations.

Why we inspected

We carried out an announced comprehensive inspection of this service on 11 March 2020. A breach of legal requirements was found. The provider completed an action plan after the last inspection to show what they would do and by when to improve Good Governance.

We undertook this focused inspection to check they had followed their action plan and to confirm they now met legal requirements. This report only covers our findings in relation to the Key Questions Safe and Well-led which contain those requirements.

The ratings from the previous comprehensive inspection for those key questions not looked at on this occasion were used in calculating the overall rating at this inspection. The overall rating for the service has changed from Requires Improvement to Good. This is based on the findings at this inspection.

During a routine inspection

About the service

Elite Home Care Solutions (UK) is a domiciliary care agency providing personal care to people in their own homes. Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do we also consider any wider social care provided. At the time of inspection 50 people were receiving personal care.

People’s experience of using this service and what we found

Medicines were administered safely to people, however the recording of medicines needed to be clearer. Returned medicines did not have a clear audit trail in place to record this practice.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; however, the documentation in the service did not support this practice. We made this a recommendation.

Whilst the service carried out quality monitoring in practice there were no formal documented audits of the service. Records were not always accurate and complete. This was a breach of Regulation 17 (Good governance).

Records of mental capacity assessments had not been completed for people who needed support to make decisions.

Everyone we spoke with told us they felt safe. The service had enough staff to safely support and meet people’s needs.

People were supported by staff that knew them well and picked up quickly on any changes in how people presented.

People’s relatives praised the kindness, care and consistency the staff provided.

Although the service was not set up to provide activities to people, they still made efforts to involve people and bring them together.

Everyone we spoke with was very complimentary about registered manager and said that they would or had already recommended the agency to other people. There was a mutual appreciation between staff and the registered manager. Staff told us they were well supported, and communication was good within the team.

During a routine inspection

Elite Home Care Solutions (UK) provides care to people in their own homes in Gloucester and the surrounding areas. The service is provided to people who have a range of needs including physical disabilities and age related frailty. At the time of our inspection, 60 people were receiving a service from Elite Home Care Solutions.

This inspection took place on 24 and 25 April 2017. The service had not previously been inspected.

There was a registered manager at the service. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

People received a service that was safe. Where people needed help with medicines, the administration, storage and recording of medicine was safe. People were safe from harm because staff were aware of their responsibilities and, knew how to report any concerns. There were enough skilled and experienced staff to safely provide care. Recruitment checks were carried out before staff worked with people to ensure they received care from suitable staff. Risks to people were assessed and action taken to manage these.

People received effective care and support. Staff received the training and support required to effectively meet people’s needs. The registered manager and staff understood the principles of the Mental Capacity Act (MCA) 2005 and, worked to ensure people’s rights were respected. Where people required, staff supported them to eat and drink. Staff ensured people received assistance from other health and social care professionals when required.

People received a service that was caring. People received care and support from caring and compassionate staff who knew them well. Staff provided the care and support people needed and treated them with dignity and respect. People and, where appropriate, their families were actively involved in making decisions about their care and support.

The service was responsive to people’s needs. People received person centred care and support. The service listened to the views of people using the service and others and made changes as a result. People were supported to participate in a range of activities based upon their assessed needs and wishes.

There was a registered manager in post who offered good leadership to ensure the service was well-led. Quality checks were in place and these identified areas of good practice as well as areas for improvement. Where issues had been identified, there was a clear action plan to address them. The registered manager had clear visions and values for the service and, had communicated these effectively to people, their relatives, staff and other health and social care professionals. Staff, people and their relatives spoke positively about the manager.
