During an inspection looking at part of the service

We undertook a follow up inspection of Poringland Dental Surgery on 18 April 2023. This inspection was carried out to review in detail the actions taken by the registered provider to improve the quality of care and to confirm that the provider was now meeting legal requirements.

We had previously undertaken a comprehensive inspection of the practice on 13 July 2022 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We found the registered provider was not providing well-led care and was in breach of regulations 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. When one or more of the five questions are not met, we require the service to make improvements and send us an action plan. We then inspect again after a reasonable interval, focusing on the areas where improvement was required.

As part of this inspection, we asked:

• Is it well-led?

Our findings were:

We found this practice was providing well-led care in accordance with the relevant regulations. The provider had made adequate improvements in relation to the regulatory breach we found at our previous inspection. In general improvements were noted in staff recruitment, medical emergency equipment and infection control.

There were areas where the provider could make improvements. They should:

  • Take action to ensure that clinicians follow guidelines issued by the FGDP for antimicrobial prescribing in dentistry.

  • Take action to ensure all loose and uncovered items in treatment room drawers are covered to prevent aerosol contamination.

Background

Poringland Dental Practice provides private dental care and treatment for adults and children.

There is level access to the practice for people who use wheelchairs and those with pushchairs. There are ground floor surgeries and a fully accessible toilet.

Car parking spaces are available on site.

The dental team includes 3 dentists, 3 dental nurses, a receptionist, and a practice manager. The practice has 3 treatment rooms, only 2 of which were in use at the time of our inspection.

During the inspection we spoke with the practice manager, a dental nurse and 2 dentists. We looked at practice policies and procedures and other records about how the service is managed.

The practice is open on Mondays to Fridays from 9am to 5.30pm.

During an inspection looking at part of the service

We carried out this announced focused inspection on 13 July 2022 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered practice was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a Care Quality Commission, (CQC), inspector who was supported by a specialist dental adviser.

We usually ask five key questions, however due to the ongoing COVID-19 pandemic and to reduce time spent on site, only the following three questions were asked:

• Is it safe?

• Is it effective?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

  • The dental clinic appeared to be visibly clean and well-maintained.
  • Safeguarding processes were in place and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • Patients were treated with dignity and respect and staff took care to protect their privacy and personal information.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • Staff felt involved and supported and worked well as a team.
  • Staff and patients were asked for feedback about the services provided.
  • The dental clinic had appropriate information governance arrangements in place.

Appropriate pre-employment references had not been obtained for new staff.

  • Auditing and risk management systems within the practice were not effective in driving improvement.
  • Some of the practice’s infection control procedures did not take into account the guidelines issued by the Department of Health in the Health Technical Memorandum 01-05: Decontamination in primary care dental practices.

We identified regulations the provider was not complying with. They must:

Establish effective systems and processes to ensure good governance in accordance with the fundamental standards of care. Full details of the regulation the provider was not meeting are at the end of this report.

There were areas where the provider could make improvements. They should:

  • Implement a system to ensure patient referrals to other dental or health care professionals are centrally monitored to ensure they are received in a timely manner and not lost.

  • Improve and develop staff awareness of the requirements of the Mental Capacity Act 2005 and ensure all staff are aware of their responsibilities under the Act as it relates to their role.

During a routine inspection

We spoke with two people who were attending the practice. They both told us they were always given a copy of their treatment plan that showed the cost of the treatment. They told us they felt respected by the staff and were fully involved in the decisions about their dental care and treatment.

People were complimentary about the dental care and treatment they received and one person told us: "I travel quite a way to see my dentist, because I have always been satisfied with the care and treatment I get here, the staff are so friendly and the place has a good atmosphere."

Another person told us:"I have been coming here for years, the staff know me. My dentist may have changed but I know they are a reliable team and I don't feel so anxious coming to the dentist anymore."

The practice had three surgeries that were accessible for people with mobility difficulties. The practice manager made available all the records we asked to see. They had systems in place to monitor and reduce cross infection, safeguard vulnerable people and investigate complaints. There was the opportunity for people to air their views through the patient satisfaction survey and leave their comments in the suggestion box. We saw that the information gathered was used to improve quality of care.
