Assessing needs

Score: 3

The registered manager assessed people’s needs before they started using the service. They involved the person, their relatives and other professionals. Staff carried out regular reassessments, helping them to identify changes. Relatives told us they were consulted and understood how care needs were assessed and planned for. Staff created person-centred care plans which set out how people wanted and needed to be cared for.

Delivering evidence-based care and treatment

People received evidence-based care and support. Staff completed a range of training relevant for their role. This included training about people with a learning disability. The provider sourced an external professional to help train staff, and to create support plans. They helped staff follow best practice and to understand how people communicated their needs. They developed proactive strategies for staff to follow to reduce people’s anxiety and distress.

The provider shared information about best practice guidance and legislation with staff. Staff explained they had undertaken a range of qualifications and training which helped them understand about people’s needs.

How staff, teams and services work together

Staff worked well together, sharing information to help ensure people received consistent care and support. They took part in regular meetings with each other and managers. They had effective systems for sharing written information. Staff worked closely with external healthcare professionals, making timely referrals when they identified a need.

Supporting people to live healthier lives

People were supported to live healthy lives. Staff assessed and planned for people’s healthcare needs. Staff had information about different healthcare conditions, including symptoms and signs of deteriorating health. People were supported to attend regular health appointments. Staff shared information with professionals to help make sure people received the right care and support. Guidance and information from health professionals were included in care plans.

Staff supported people to exercise and to eat well, making healthy choices about their lives.

Monitoring and improving outcomes

Staff monitored people’s needs and outcomes. They regularly reviewed people’s care plans. People were supported to set personalised objectives and work towards these. Staff monitored people’s health, nutritional intake and hydration to make sure they were receiving the right support to stay well.

The provider assessed people’s mental capacity to make different decisions. People using the service did not use many words to communicate. Staff considered their individual communication needs and tried to present choices in a way people could understand. People lacked the mental capacity to make complex decisions. Staff liaised with people’s representatives to discuss the best interests for each person with different decisions. Staff completed training to understand about their legal responsibilities and were able to explain these to us.

Relatives told us they were involved in important decisions. They explained that staff provided stable consistent care which ensured people understood what was happening and felt prepared and safe.
