Livingways Care Services Ltd (Carlisle) is a domiciliary care agency providing personal care to younger and older adults, some of whom may be living with dementia, mental health needs, have a physical disability or a sensory impairment. The service also has the service user band for children 0 – 18. The provider confirmed they do not support children, we have explained how they could apply to remove this.

Not everyone who used the service received the regulated activity of personal care. CQC only inspects where people receive personal care. At the time of the assessment there were 54 people receiving a regulated activity. This was the first assessment of the service and was completed using remote inspection methodology.

There was a positive culture which focused on delivering safe, caring and person-centred care.

Staff understood safeguarding procedures and knew how to raise concerns. Incidents and accidents were recorded, reviewed and analysed to identify learning and reduce the risk of recurrence.

Staff worked with health and social care professionals to support people’s health and wellbeing and to respond appropriately when people’s needs changed.

Systems were in place to assess people’s needs and plan their care. People were involved in decisions about their care and support, and staff promoted independence, choice and control.

Some improvements were needed. We identified inconsistencies and contradictions in some care plans and medicines records. There was not always clear guidance on how to support people with personal care and moving and handling. Governance systems were in place but were not always effective in identifying these issues proactively. Some people and relatives described a lack of continuity of care and communication challenges, particularly where multiple carers were involved or where language barriers existed.

The registered manager responded promptly to the issues identified during the assessment and took immediate action to review care plans, strengthen audits and address training needs. Leaders were visible and approachable, and staff described a supportive culture where they felt able to speak up.

People's experience of the service

People and relatives were positive about the care and support they received.

Staff were described as kind, caring and respectful. One person said, “The carers are very nice and they chat – we have a lot of banter going on and they never rush me.”

People, and their relatives said staff made them feel safe and well supported in their own homes. One person commented, “I would be lost without them. They keep me safe and help me with anything I need.”

People valued being able to remain living independently and spoke positively about staff responding appropriately when health needs changed, including seeking medical advice and keeping relatives informed. A relative told us, “They always let me know if they have phoned the doctor.”

Medicines were generally managed safely and in line with guidance.

People were involved in decisions about their care and said staff were flexible and responsive to requests for changes. Staff were described as friendly and unhurried, and people told us their privacy and dignity were respected. Most people experienced positive relationships with carers they knew well. One person said, “I do know my carers mostly and that’s really nice.”

A small number of people and relatives said that greater continuity of staff and clearer communication would improve their experience. One relative said, “We need more continuity of care… we never know who is coming."
