During an assessment under our new approach

Date of Assessment: 5 August to 21 August 2025. Wellspring Surgery delivers services to 10,515 people under a contract held with NHS England. The provider of the service is the Modality Partnership which operates GP health care services and community services nationally. The ethnic make-up of the practice area were 60.74% white, 7.36% mixed, 10.63% Asian, 16.63% Black and other 4.63%. The age distribution of the practice population shows they have a lower proportion of older people and higher proportion of working age and young people, in comparison to local and national averages. Information published by Office for Health Improvement and Disparities shows that deprivation within the practice population group were in the 1st decile (1 of 10). The higher the decile, the least deprived the practice population were relative to others. This assessment considered the demographics of the people using the service, the context the service was working within, and how this impacted service delivery. People were protected and kept safe. The service had a good learning culture and people could raise concerns. Staff managed medicines well and involved people in planning any changes. People were involved in assessments of their needs, and care was based on latest evidence and good practice. Staff made sure people understood their care and treatment to enable them to give informed consent. People were treated with kindness and compassion. People were involved in decisions about their care including understanding options around choosing to withdraw or not receive care. The service provided information people could understand. Leaders and staff had a shared vision and culture based on listening, learning and trust. Leaders were visible, knowledgeable and supportive, helping staff develop in their roles. Staff understood their roles and responsibilities. The service supported staff wellbeing. There was a culture of continuous improvement with staff given time and resources to try new ideas.
