During an inspection looking at part of the service

We carried out this announced inspection on 21 July 2021 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered provider was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a Care Quality Commission, (CQC), inspector who was supported by a specialist dental adviser.

To get to the heart of patients’ experiences of care and treatment, we asked the following questions:

• Is it safe?

• Is it effective?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

We found this practice was providing safe care in accordance with the relevant regulations.

Are services effective?

We found this practice was providing effective care in accordance with the relevant regulations.

Are services well-led?

We found this practice was providing well-led care in accordance with the relevant regulations.

Background

Golborne Dental Care is in North Kensington in the Royal Borough of Kensington and Chelsea and provides NHS and private dental care and treatment for adults and children.

The practice is on the ground floor and in the basement of the building, and has three treatment rooms and a separate decontamination room.

There is level access to the practice for people who use wheelchairs and those with pushchairs. The practice is located close to public transport services and car parking spaces are available near the practice.

The dental team includes three dentists, two qualified dental nurses, two trainee dental nurses, one receptionist and a practice manager who is one of the qualified dental nurses.

The practice is owned by an individual who is the principal dentist there. They have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated regulations about how the practice is run.

During the inspection we spoke with the principal dentist, one dental nurse, the receptionist and the practice manager. We looked at practice policies and procedures and other records about how the service is managed.

The practice is open Monday to Friday from 9.00 am to 6.00 pm and Saturday by appointment only.

Our key findings were:

  • The practice appeared to be visibly clean and well-maintained.
  • The provider had infection control procedures which reflected published guidance.
  • There were systems in place to reduce the risks associated with the transmission of Covid-19.
  • Staff knew how to deal with emergencies. Appropriate medicines and life-saving equipment were available.
  • The provider had systems to help them manage risk to patients and staff.
  • The provider had safeguarding processes and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • Staff recruitment procedures reflected current legislation.
  • The clinical staff provided patients’ care and treatment in line with current guidelines.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • The provider had effective leadership and a culture of continuous improvement.
  • Staff felt involved and supported and worked as a team.
  • The provider asked staff and patients for feedback about the services they provided.
  • The provider dealt with complaints positively and efficiently.
  • The provider had information governance arrangements.

There were areas where the provider could make improvements. They should:

Implement audits for prescribing of antibiotic medicines taking into account the guidance provided by the Faculty of General Dental Practice.

During a routine inspection

We spoke with four people using the service. One person told us that their treatment had been "fully explained" and that they "always discuss options" with the dentists. In recent patient feedback, when asked about staff being helpful and knowledgeable and whether they received adequate information about their treatment the majority of people rated the practice as "very good".

People using the service described it as "very good" and said that medical histories were taken prior to treatment being provided. In recent patient feedback people said they were "looked after extremely well". Staff had been trained in what to do in a medical emergency. There were emergency drugs and equipment available in the practice.

On the day of the inspection the practice was clean and tidy. There were policies and procedures in place on how to maintain a clean and hygienic environment and staff had made regular checks and completed records to ensure that these were being followed.

During a routine inspection

We were unable to speak to people using the service because there were none booked at the time of our inspection. We did not try to seek information from patients at this visit as we spent time looking at the environment and infection control.

We looked at all areas of the dental practice, this included a waiting room with reception, two surgeries and a decontamination room.

On the day of the inspection the practice was not clean. There were policies and procedures in place on how to maintain a clean and hygienic environment. Staff had made regular checks and completed records about the standard of cleaning. These did not ensure that the policies and procedures were being followed.

The premise was in a bad state of repair and there were areas that required maintenance including broken tiles on the floor directly outside the two surgeries. The owner told us he had a plan to complete a thorough refurbishment of the premises and to improve the environment.

We looked at records including patient records and infection control records. All records were kept in lockable filing cabinets in the reception area that were not accessible to patients. Staff told us the computer system was password protected.
