During an inspection looking at part of the service

We undertook a follow up focused inspection of C Watson – Camberwell Road on 12 September 2022. This inspection was carried out to review in detail the actions taken by the registered provider to improve the quality of care and to confirm that the practice was now meeting legal requirements.

The inspection was led by a CQC inspector who had access to a specialist dental adviser.

We undertook a comprehensive inspection of C Watson – Camberwell Road on 26 April 2022 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We found the registered provider was not providing safe and well led care and was in breach of regulation 12 and 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. When one or more of the five questions are not met, we inspect again after a reasonable interval, focusing on the areas where improvement was required.

We found this practice was providing safe care in accordance with the relevant regulations.

Background

C Watson – Camberwell Road is in Camberwell in the London borough of Southwark and provides NHS dental care and treatment for adults and children.

There is level access to the practice for people with pushchairs, however the dental surgery is not accessible for those in wheelchairs. The provider has details of other local services they can access. Car parking spaces are available near the practice for a fee. Local transport services including bus and rail services are close by.

The dental team includes a principal dentist, a practice manager, a receptionist, a trainee dental nurse. At the time of the inspection the practice was also using locum dental nurses regularly. The practice has 1 treatment room.

During the inspection we spoke with the principal dentists and the trainee dental nurse. We looked at practice policies and procedures and other records about how the service is managed.

During an inspection looking at part of the service

We carried out this announced focused inspection of C Watson – Camberwell Road on 26 April 2022 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered practice was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a Care Quality Commission, (CQC), inspector who was supported by a specialist dental adviser.

To get to the heart of patients’ experiences of care and treatment, we usually ask five key questions, however due to the ongoing COVID-19 pandemic and to reduce time spent on site, only the following three questions were asked:

• Is it safe?

• Is it effective?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

  • Staff provided preventive care and supported patients to ensure better oral health.
  • The appointment system took account of patients’ needs.
  • The clinical staff provided patients’ care and treatment in line with current guidelines.
  • Patients were treated with dignity and respect and staff took care to protect their privacy and personal information.
  • The dental clinic had information governance arrangements.
  • The practice infection control procedures were not in accordance with published guidance.
  • The practice dd not have effective arrangements for dealing with medical emergencies . Some of the appropriate medicines and life-saving equipment were not available and staff were unclear about emergency procedures.
  • The practice did not have effective systems to help them manage risk to patients and staff.
  • Effective safeguarding processes were not in place and staff were unclear as to their responsibilities for safeguarding vulnerable adults and children.
  • The practice did not have suitable arrangements to ensure the dentist had suitably trained staff to provide chairside support when treating patients.
  • There was ineffective leadership and a lack of systems to support continuous improvement.

During the inspection we spoke with the principal dentist, the receptionist and the assistant. We looked at practice policies and procedures and other records about how the service is managed.

The practice is open between 8.30am and 5pm Mondays to Fridays.

We identified regulations the provider was not complying with. They must:

• Ensure care and treatment is provided in a safe way to patients.

• Establish effective systems and processes to ensure good governance in accordance with the fundamental standards of care.

Due to the serious level of concerns we identified, we have taken urgent enforcement action and suspended the providers registration for a period of three months. We will report further when any enforcement action is concluded.

During an inspection looking at part of the service

We carried out this inspection to determine if the provider had taken action to rectify the non-compliance that was found at our inspection in May 2013.

We found that the process used for the decontamination of instruments had improved, and now met national guidelines. Regular decontamination audits were being carried out.

We saw that staff were being supported to develop and improve their skills through attendance at training courses and monthly meetings to discuss good practice.

During a routine inspection

We spoke with three people who use the service, all of whom had been coming to the practice for several years. They told us they were very happy with the service and said the dentist provided clear information about the treatment planned. We also reviewed a sample of responses to a patient questionnaire. They all gave positive feedback about the service.

People felt staff were welcoming and friendly and treated them with respect. One person said, 'The staff are always courteous and helpful.' Another person said, 'I have a phobia about dental treatment but the dentist always makes me feel comfortable and is brilliant.'

There were arrangements in place for supporting staff but we found some shortfalls in these arrangements.

There were suitable safeguarding and emergency arrangements in place in order to ensure patient safety.

During our inspection we found that there were arrangements in place for the decontamination of dental instruments. However, there was a risk that some sterile dental instruments were not being stored appropriately.
