We carried out this announced on-site inspection on 29 July 2025.
We found the practice had not met all regulations.
Staff provided c6are and treatment in line with current guidance.
They treated patients with dignity and respect and ensured access to care, support, and treatment when required.
Staff had the skills, knowledge and experience to carry out their roles.
The practice had safeguarding processes and staff knew their responsibilities for safeguarding.
The practice had processes to identify and manage risks, but improvement was needed.
Staff did not manage prescriptions in line with current guidance.
The practice is in Lydney, Gloucestershire and provides NHS and private dental care and treatment for adults and children.
There was step-free access to the practice and car parking spaces available, including dedicated parking for disabled people, near to the practice.
The practice had 2 treatment rooms.
At the time of our inspection, there was a total of 9 staff, of which there were 2 dentists, 4 dental nurses, who all covered reception desk, and 3 receptionists.
We gathered feedback from staff and spoke to a range of staff during our inspection, including 2 dentists, 2 dental nurses and a nurse who was covering reception desk.
The provider was not complying with regulation 17 Good Governance.

We have asked the provider for an action plan in response to the concerns found at this inspection.
People's experience of the service

Two weeks before our inspection, we asked the practice to encourage patients to share their views of the service with us.
We received feedback from 2 patients. On the day of our inspection, we saw patient feedback from a further 9 patients.
Patient feedback provided a positive view of the dental team and care provided by the practice. Comments included,
“I received fabulous treatment on my front tooth which was damaged due to biting into nectarine. I'd broken or chipped the tooth. I immediately called the Surgery, and they got me in the following day I cannot see where the work has been done. Thank you.”
“I had fantastic care from this practice. They took the time to answer questions, give advice and fix anything minor that was irritating me. The filling that I received was exceptional and visually undetectable. The injection was painless. The receptionists and assistant were lovely and professional. I left feeling much better about my oral health. The best NHS dental service I have received in my 40 years.”
Patients commented positively about the standards of cleanliness.
Patients felt able to book appointments within an acceptable timescale for their needs and said they had enough time during their appointment without feeling rushed.
Patients told us they were given clear information to help them make an informed choice about their treatment and any associated costs. They were involved in decisions about their care.
Patients told us that when they were prescribed medicines, sufficient information was given.
The practice shared patient feedback with the team. We were told this was reviewed and where suggestions had been made, appropriate action would be taken.
