Our view of the service

We assessed Cygnet Raglan House on 10 and 11 February 2026, with additional off-site activity following the site visit. This was a routine unannounced inspection.

Cygnet Raglan House is a 25-bed high support inpatient rehabilitation service for women. The service supports women with complex mental health needs, including those who have experienced multiple placement breakdowns, unsuccessful treatment programmes, or who are stepping down from secure services. The service is designed to promote recovery, with a focus on space, personal privacy, dignity, and independence.

  • Cygnet Raglan House is registered with the Care Quality Commission to carry out the following regulated activities:
  • Treatment of disease, disorder or injury
  • Assessment or medical treatment for persons detained under the Mental Health Act 1983
  • At the time of the inspection, the service had a registered manager. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run. We assessed the assessment service group of long stay rehabilitation mental health wards for working age adults. At this inspection, we assessed all quality statements across the safe, effective, caring, responsive and well-led key questions.

Before the inspection visit, we reviewed information we held about the provider via our internal monitoring system. During the inspection, we spoke with seven patients, 3 carers, and the registered manager. We spoke with 16 staff members, including doctors, nurses, healthcare assistants, the head of care, a psychologist, a housekeeper, occupational therapists, consultants, and a speciality doctor. Carers and commissioners provided positive feedback about the service. We reviewed 8 care and treatment records, checked medication management and prescribing practices, and examined operational policies and procedures.

This was an unannounced inspection. We last inspected Cygnet Raglan House in 2017. At that time, the service was rated good overall, with ratings of good in all domains except for well-led, which was rated outstanding. Following the previous inspection, the provider was not in breach of regulation. At this inspection, we rated the service as good overall, with ratings of good across 4 key questions except for caring which had a rating of required improvement. People were supported to have choice and control over their care and treatment and were involved in planning their care. Feedback from patients and carers was actively sought and used to make improvements to the service where appropriate. Care plans and risk assessments supported safe practice and were reviewed regularly. Leaders had developed partnerships within the local community which supported patients’ wellbeing and access to meaningful activities. However, we identified 1 breach in the legal regulation of Respect and Dignity Regulation 10 (1) (2)a. The provider did not ensure that all service users were always treated with dignity and respect. This was reported by all 7 patients we spoke with and observed by inspectors during our assessment.

We have asked the provider for an action plan in response to the concern found at this assessment.
