At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people’s needs were met through good organisation and delivery.

Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. Records evidenced that where people’s needs had changed the service made prompt and appropriate referrals to partner agencies. Regular review meetings were held with people, their families and professionals to ensure care remained in line with people’s needs.

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. The provider ensured that staff received the correct training and had the appropriate skills to meet the needs of the people they supported. The provider had worked to improve people’s care plans and assessments which had improved since the last inspection.

Providing Information

The provider did not always supply appropriate, accurate and up-to-date information in formats that were tailored to individual needs. Improvements had been made at the service in relation to record keeping and information sharing, however there were areas where further improvements were required. For example, a person was not told about a scheduled meeting that was arranged for them and a professional told us information was not passed to them in a timely manner. Although records evidenced most information sharing was in line with expectations, further work was required to achieve consistency in this area. The registered manager demonstrated a commitment to ongoing improvement in this area.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result. Staff demonstrated a commitment to involving people in day-to-day decisions, for example a staff member showed us picture cards in the kitchen used to support people to make choices around meals. Most feedback in relation to meals and choice at the service was positive.

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it. Records evidenced appropriate referrals and appointment were made when required.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. People were supported to work towards their goals. For example, 1 person had improved their language skills at the service and was supported on regular trips out into both the local community and day trips including the seaside.

Planning for the future

Score: 3

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future. People were supported to make plans for the future and work towards these. Record keeping at the service had improved and this enabled staff and managers to assess what did and did not work for people and to adapt their approach accordingly.
