During an assessment under our new approach

Date of assessment: 16 June to 24 July 2026. Total Quality Care Services Ltd is a care at home service providing support to people living in their own homes. The Care Quality Commission (CQC) only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do, we also consider any wider social care provided. The service supports older people, younger adults, people living with a mental health diagnosis, physical disabilities and people living with a learning disability and/or autism. At the time of our assessment the service was providing support to 7 people.

We assessed the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed people with a learning disability and autistic people respect, equality, dignity, choices, independence and good access to local communities that most people take for granted.

The assessment was due to an aged rating. We have assessed the service against all the quality statements in the key questions of safe, effective, caring, responsive and well-led.

The provider’s governance systems were not always effective in identifying issues related to staff recruitment and training and up to date information in respect of people’s care needs. The provider had responded promptly to the areas we had identified as requiring attention.

Staff told us they had received the necessary training to support them in their role. However, there were gaps in training for some specific health conditions such as catheter care, diabetes awareness and epilepsy awareness. The provider took immediate action to make sure all staff were registered to complete these training modules and provided confirmation of the oversight that this would be completed. All staff had completed their safeguarding training and were aware of the steps to take to protect people from the risk of abuse. Staff received supervision and told us they felt supported by the provider. There were enough staff available to support people.

There were no concerns identified with infection control practices.

Staff understood the importance of supporting people’s independence and gaining people’s consent and involved people in decisions about their care. Staff protected people’s privacy and dignity and treated them as individuals supporting their individual preferences.

The provider told us they had processes in place to investigate incidents, accidents, complaints and safeguarding concerns. We were able to see these concerns had been investigated thoroughly and any learning had been shared with the staff.

Staff told us they felt able to give feedback to the provider and were treated well, free from bullying or harassment. Staff told us they understood their roles and responsibilities. The provider took a positive approach to our assessment and considered it an opportunity for continued learning to make improvements.

During a routine inspection

The inspection took place on 22 August 2017 and was announced. We gave the service 48 hours’ notice of the inspection because we wanted to ensure that the registered manager was available. This was the first inspection of this service since it registered with us on the 07 January 2015. This is because although registered with us no one was using the service until 2017.

Total Quality Care is registered to provide a personal care service to adults in their own homes. At the time of the inspection three people were using the service. The service is a family run service employing three staff and supporting three people. On occasions the registered provider/manager would also undertake care calls.

There was a registered manager in post who was also the provider. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act (2008) and associated Regulations about how the service is run.

The registered manager and staff were knowledgeable about how to keep people safe and recognized the different forms of abuse and the action to take if required, in order to keep them safe.

The registered manager and staff had the necessary skills and knowledge to meet people’s individual needs.

People received the support they required with their diet. Other health services were advised of changes in people's health and circumstances by family members.

People's rights were protected and their consent was always sought before and during care. The service was delivered in a way that enabled people to make decisions on how they were supported and their wishes were met according to their preferences and choices.

The registered manager and staff had undertaken appropriate training so they would know how to support people who lacked capacity to make decisions for themselves. People’s dignity, privacy and independence was respected.

People were involved in the planning, assessment and review of their care and knew who to contact if they were unhappy about any aspect of their care. There was a system in place to manage complaints.

Systems were in place to monitor the service to ensure people received quality care.
