Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. Care plans were detailed and included medical history, preferences, and risk assessments. For example, 1 person’s plan included spinal injury-related needs, continence support, and emotional wellbeing. Another plan incorporated sensory sensitivities and communication preferences. Staff used tools like the MUST (to ensure people were not at risk of malnutrition) screening and Waterlow score to assess nutrition and skin integrity risks.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. Care and treatment were based on best practice and clinical guidance. Staff were trained in complex care areas such as spinal injury, autonomic dysreflexia, bowel care, and assisted cough techniques. The training workbook covered clinical observations and procedures, with nurse sign-off required before staff could deliver care independently. One staff member said, “I’ve had training in PEG, nebulisers, and diabetes, it’s helped me progress and support people better.”

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. For example, feedback confirmed that carers kept families informed and collaborated with healthcare teams.

Supporting people to live healthier lives

Score: 3

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Nutrition and hydration were monitored, with carers prompting people to drink and assisting with meal preparation. One person said, “They help me plate food and carry drinks. I like toast and strong tea.”

Risks such as dehydration and malnutrition were identified and managed. Staff supported people to maintain independence, with 1 person saying, “I want to do things myself, they only help when I ask.”

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. Reviews and quality checks where routinely carried out to ensure people’s care packages were meeting their needs. One relative discussed with us, how following a review, their relatives call time was moved to suit them better.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. For example, 1 person’s plan noted they had capacity but preferred to ask a family member for guidance. Staff understood the importance of consent, with one saying, “We always ask before doing anything, it’s about respecting people’s choices.”
