Governance, management and sustainability
Oversight of the service was poor and ineffective. The registered manager had not completed audits to assess the quality of the service which would have identified the concerns highlighted within this assessment. There was no system to review care plans and ensure they were sufficiently detailed and contained key information and guidance for staff. A medication audit was completed but this only reviewed if medication had been administered. We found in the medication risk assessment, it was noted that the medication had not been stored appropriately, but this has not been actioned or followed up in the audit. The provider was not working in line with their care planning policy. The policy states, “The Client’s Care Plan should be generated from performing a needs assessment exercise with the Client and, wherever possible, should cover all aspects of each Client’s personal, social and health care plans.” During our conversations, we found the registered manager knew the person they were supporting well but certain information and detail was lacking from a person’s care plan, for example a person who required to be hoisted out of bed was not included in their care plan. The registered manager states this was because they only supported this person during the night, but this did not take into account any emergencies that could occur and how the person needed to be transferred out of the bed. During our assessment, we found inaccuracies in the registered managers training documents, this was raised with the manager on separate occasions who stated this was an oversight issue. The registered manager discussed during the assessment that they had an action plan in place to drive improvements. The action plan shared with us only focused on getting more people to support.
