People's current needs and risks were not always clearly assessed. Care plans did not always contain enough information to guide staff on how to support people safely. Risk assessments related to people’s health and safety lacked sufficient detail and guidance for staff to follow to help keep them safe. This included risks related to medication. One person’s care plan identified they were at risk of aspiration but there was no risk assessment to try and mitigate this risk and what staff would need to look out for that would indicate concerns to a person’s health. This meant staff did not have guidance on how to support people safely and people were at risk of avoidable harm. Care plans we reviewed were not sufficiently detailed to inform staff how best to support people with their needs. For example, there was a lack of detail around how to support people with their personal care needs. We also found there was no medication guidelines, and the care plan did not provide sufficient detail on how a person should be supported with their medication. One medicine administration record (MAR) lacked instructions on how medication should be administered. We also found there was insufficient detail recorded around supporting a person with tube feeding. However, the registered manager knew people and their needs well, so we did not find any harm was caused because of this. A relative confirmed the registered manager knew how to manage their relative’s risks safely.
