Assessing needs

The provider did not always ensure people's assessed needs were kept under review and accurately reflected in care records. Assessments had not always been updated when people’s needs changed. For example, the care for 1 person had been adjusted to meet a specific need. However, their assessment and care plan had not been updated. We discussed this with senior manager who agreed to ensure care records were updated in a timely way.

People’s needs were assessed before care commenced. Managers considered people's health conditions, communication needs, cultural preferences and support requirements when planning care. They developed care plans that reflected people's individual needs and provided guidance for staff.

Delivering evidence-based care and treatment

The provider always planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. Staff received training relevant to their roles. Staff demonstrated a good understanding of people's needs and preferences. Relatives told us staff were professional and knowledgeable. Care records showed people received support in line with their assessed needs.

How staff, teams and services work together

The provider did not always work effectively across teams to support people. Formal supervision and team meeting arrangements were not in place. As a result, records did not consistently demonstrate how staff were supported to discuss their practice, learning, development needs, or the care provided to people. We discussed this with a senior manager, who described the support available to staff and agreed to review how these arrangements could be formalised.

Staff worked closely with relatives and healthcare professionals. Information was shared appropriately when people's needs changed. Staff told us managers were accessible and provided support when required. One staff member said, "If I face any problem, they will give me a solution."

Supporting people to live healthier lives

Staff monitored people’s health and wellbeing and acted when concerns were identified. Records showed concerns were escalated appropriately and shared with relevant professionals and relatives where required. Managers supported people to access healthcare services and other support when needed.

Monitoring and improving outcomes

The provider routinely monitored people's care and treatment to support positive outcomes. Daily records provided clear information about people's care, health and wellbeing, and any changes in their needs. Staff recorded concerns and the actions taken in response. Managers regularly reviewed care records to maintain oversight of care delivery, identify concerns and ensure appropriate action was taken where required.

People consented to their care and treatment. People were supported to make decisions about their care. Managers conducted mental capacity assessments and made best-interest decisions when required. Staff understood the importance of obtaining consent before providing support.
