Safe
Our overall rating of safe at Acquiesce 2 was good.
There was a proactive, systematic approach to managing safety. Leaders embedded, maintained and sought to continuously improve a culture of openness and collaboration, and safety was everyone’s top priority. Staff demonstrated a commitment to improve safety and there were clear roles, responsibilities and structures to meet safety goals.
There was proactive identification of hazards and risks, which were robustly assessed and managed to avoid adverse outcomes. Solutions to risks were developed collaboratively with the right people and the effectiveness of the controls was monitored and measured.
Staff were actively encouraged and appreciated for raising concerns about safety and ideas to improve. People felt part of a team where they could contribute towards safety and improvement. The primary response was always to learn and continuously improve.
Leaders ensured there were enough skilled staff to deliver safe care and could demonstrate adaptive strategies for responding to demand and capacity issues. Training included safety expertise, human factors and communication to support a positive safety culture within all levels of the workforce.
Services were planned and organised with people and communities, including those from minority groups and those where outcomes were known to be poorer. This was done in a way that improved their safety across care journeys, and safety was monitored collaboratively.
Information and intelligence were actively sought out to demonstrate that people were always safe and protected from bullying, harassment, avoidable harm, neglect, abuse and discrimination. Their liberty was protected where this was in their best interests and in line with legislation.
Clients were given information and leadership skills supported them to make choices which balanced risks of harm with positive choices about their lives.
