This assessment was prompted in part by concerns about the care and safety of people living at Napier Lodge. We examined those risks.
The service is a care home supporting people with nursing, physical and mental health needs.
We found 6 breaches of regulation in relation to person-centred care, dignity and respect, consent, the management of risk and medicines, safeguarding people from abuse and good governance.
Leadership and governance arrangements had not ensured people received safe, high-quality care and person-centred care. People had not been protected from the risk of abuse and harm; and had been treated with a lack of dignity and respect. People’s choices had not been followed, and actions had not always been taken in people’s best interests. People did not always receive their medicines as prescribed, and risks associated with their needs had not been safely managed. There was a lack of suitably skilled and competent staff.
When we inspected, we noted the provider was aware of the need to make significant improvements in the service and had both changed and increased the level of leadership in the service. They were also being supported by partner agencies such as the local authority Care home quality improvement team. This team provides additional nursing expertise and clinical leadership to support care homes to maintain the health of the people who live there.

This service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.
Shortly after our inspection we were advised that, due to the ongoing concerns in the home, the provider had made the decision to close Napier Lodge. A plan was put in place, supported by the local authority, to enable people to have a safe and smooth transition.
