During an inspection looking at part of the service

About the service

Sanctuary Oasis Limited is a domiciliary care service. The service provides care and support to people living in their own homes. At the time of the inspection there were 29 people using the service.

Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do, we also consider any wider social care provided.

People’s experience of using this service and what we found

People’s safety was promoted. Staff were trained and understood the actions needed to minimise the risk of avoidable harm, abuse and the prevention of avoidable infection. The registered manager had worked in partnership with health care professionals that promoted good outcomes for people.

People felt safe with the staff and were happy with the care and support they received. People were supported by a small team of staff, which provided consistency and continuity of care.

Potential risks to people’s safety had been assessed, managed and was regularly reviewed. Care plans provided clear guidance for staff to follow which included instructions from health care services to manage identified risks such as pressure care. Staff supported people with their medicines safely.

The provider continued to follow safe staff recruitment processes. Staff were committed to providing high-quality service. They had undertaken training for their role to they were skilled and knowledgeable to effectively meet people’s needs.

People and relatives all agreed that the registered manager was responsive, approachable and were confident concerns would be addressed.

The provider had systems in place to monitor and improve the quality and safety of the service provided. People’s views about the service were sought individually and through quality assurance surveys.

The registered manager was aware of their responsibilities in meeting their legal obligations. They kept their knowledge up to date and were part of the local social care forums. Health and social care professionals found the registered manager was responsive and committed to a providing a service were people’s safety and independence was promoted.

Rating at last inspection

The last rating for this service was good (report published 26 July 2019).

Why we inspected

We received concerns about the management of risks in relation to pressure area care and record keeping. As a result, we undertook a focused inspection to review the key questions of Safe and Well-led only.

We reviewed the information we held about the service. No areas of concern were identified in the other key questions. We therefore did not inspect them. Ratings from previous comprehensive inspections for those key questions were used in calculating the overall rating at this inspection.

We found no evidence during this inspection that people were at risk of harm from this concern. The overall rating for the service has remained ‘Good’. This is based on the findings at this inspection. Please see the Safe and Well-Led sections of this full report.

Sanctuary Oasis Limited is a domiciliary care agency providing personal care to a range of people living in their own homes. At the time of the inspection 27 people were receiving personal care.

Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do we also consider any wider social care provided.

People’s experience of using this service and what we found.

People told us they received safe care. Staff fully understood safeguarding procedures. Risks within people’s lives were assessed to ensure people and staff providing their care were safe and protected from harm.

Staff recruitment procedures ensured that appropriate pre-employment checks were carried out. The staffing levels matched people’s assessed support needs.

Staff received induction training and ongoing refresher training to keep up to date with current good practice guidelines. Staff received suitable supervision and support and felt confident in their roles. The staff felt well supported by the registered manager.

People were supported to eat and drink enough to meet their nutrition and hydration needs. Staff closely monitored any changes in people’s health and well-being and supported people to access to health professionals as required.

Staff treated people with kindness, dignity and respect. The care plans reflected people’s likes dislikes, needs and preferences. People and their representatives were involved in planning their care as much as possible.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

The provider had a complaint procedure, which was used effectively. They were open and honest, and worked in partnership with outside agencies. The registered manager understood their regulatory responsibilities. The rating from the previous inspection was displayed within the service and on the providers website. Notifiable events had been submitted to the Care Quality Commission (CQC) as legally required.

1 November 2016

During a routine inspection

This inspection took place on the 1 November 2016, with an announced visit to the service. In addition, phone calls were made to people using the service and their relatives on the 3, 7 and 10 November 2016.

Sanctuary Oasis provides personal care for people living in their own homes in Milton Keynes and Northamptonshire. When we inspected they were providing care for approximately 21 people.

The service had a registered manager in post. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are 'registered persons'. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

Systems were in place to identify potential risks to people, and to put steps in place to help staff to effectively manage the risks. People felt safe when they received care from the service and staff worked to protect them from harm or abuse.

Staffing levels were consistent and sufficient to meet people's needs. Pre-employment checks were carried out on all staff before they started working at the service to ensure they were of good character and suitable for their roles.

Staff received the training and support they needed to make sure they had the knowledge and skills to carry out their roles. Staff supervision systems were in place to provide a forum for staff to discuss their learning and development needs and any concerns they had.

Consent to care and support was sought from people using the service. Where it was assessed that people lacked the mental capacity to give their consent or make specific decisions, best interests’ decisions had been made by their family members or their close relative. Staff supported people to maintain a healthy and balanced diet and to seek the support of healthcare professionals when needed.

People were treated with kindness and compassion. Positive caring relationships were developed between people using the service, their relatives and staff. The staff ensured that people’s dignity was maintained at all times. People’s specific needs and preferences were respected by staff when providing their care. People and their relatives were involved in care assessments and on-going reviews of their care needs.

Systems were in place to routinely listen and learn from people’s experiences. People and their relatives were encouraged to make a complaint if they were dissatisfied with any aspect of the care they received from the service.

There was a positive and open culture at the service. Quality assurance checks were regularly carried out by the provider. The feedback received from people using the service and their relatives, was used to identify areas where the service was doing well and areas to drive further improvement.
