Date of assessment: 4 June and 5 June 2026.

Sylvian Care Farnham is a domiciliary care agency. The service is registered to support younger and older adults, those with mental health conditions, physical disabilities and people living with dementia.
This was the first assessment of this newly registered service. At the time of the inspection there were 6 people being supported with personal care. As part of this assessment we also reviewed information in the ‘Provider Information Return’ (PIR). This is a comprehensive, structured self-assessment document that providers must submit to the CQC.

People were protected from abuse and avoidable harm. The provider had a positive learning culture and systems to record, investigate and respond to accidents and incidents These systems were used effectively, and lessons were shared with staff to support continuous learning and improvement. Leaders and staff understood their responsibility to report and investigate safeguarding concerns.

Care and support were effective and outcomes were consistently positive. People’s needs were assessed comprehensively before care commenced and were reviewed regularly to ensure support remained appropriate and responsive to change. Staff worked in line with evidence‑based practice and in partnership with health and social care professionals to ensure joined‑up care and continuity.

Leaders had clear governance arrangements and used audits, feedback and performance information to monitor quality and drive improvements. The culture was open, inclusive and supportive, with a strong focus on staff wellbeing and development.

People's experience of the service

People using the service experienced care that was kind, compassionate and consistently focused on what mattered to them. People told us they were treated with dignity and respect and felt comfortable and safe with staff who supported them in their own homes. People described positive, trusting relationships with staff, which helped them feel confident and well‑supported.

People and relative felt that staff were sufficiently trained and understood the risks relating to people’s care. They said staff also attended the care visit and were notified if staff were going to be late.

People felt listened to and knew how to share feedback or raise concerns. Leaders used regular surveys and day‑to‑day conversations to gather people’s views and demonstrated that feedback was acted upon to improve the service. Information was shared in accessible formats, helping people understand their care and stay informed about the service.
