During an inspection looking at part of the service

When we visited the service in May 2013 we asked the provider to take action to improve the service. In November 2013 we made a follow-up visit. We found that effective action had not been taken. The required standards were not met. We issued a formal warning that improvements must be made by 11 January 2014.

On our last visit we found that the service had not kept people's records up to date. This placed them at risk of receiving unsafe care and treatment. On this visit we found improvements had been made. People's records were accurate and fit for purpose.

Previously we found the provider did not monitor the quality of the service in relation to the current required standards. On this visit we found appropriate checks were being made to identify and address risks to people's health, safety and welfare. There was an effective system to regularly assess and monitor the quality of service people receive.

When we last visited the service we found that improvements were required in relation to the maintenance of the bathroom. On this visit we found that effective action had been taken. The provider had taken steps to ensure people were supported in suitable premises.

During an inspection looking at part of the service

At our last inspection in May 2013 we asked the provider to improve how they maintained people's records. At this inspection, we were checking to see if the provider had made the required improvements. On this inspection we found that the information kept in relation to people's care and support was not accurate and did not contain all the appropriate information. This meant that people were at risk of receiving unsafe care and support that did not fully meet their needs. Action had not been taken on the issue we previously identified. The provider needs to make improvements to become compliant with the standards.

Because of our previous concerns about the service, on this inspection we checked the provider's arrangements to ensure that it met the current standards for a care home for people with learning disabilities. The provider did not have an effective system in place to regularly assess and monitor the service with regard to 'Care Quality Commission: Guidance about compliance, Essential standards of quality and safety, March 2010.' This meant that, for example, they did not have information on the service's arrangements to ensure people's dignity, privacy and independence. Nor did they receive information about the maintenance of the building or the quality of record keeping. The provider needs to improve how they assess and monitor service provision in order to meet the standard.

At the inspection in May 2013 we found the premises were clean and well-furnished. On this visit we found that the building had been suitably maintained with the exception of the bathroom. Fittings in the bathroom had not been maintained to an adequate standard. This meant it was possibly unhygienic and uncomfortable for people to use. We have asked the provider to take action on this.

During a routine inspection

People's views and experiences were taken into account in the way the service was provided and delivered in relation to their care. People told us that they are consulted by staff and the manager about their care and prior to any changes being made.

People experienced care, treatment and support that met their needs and protected their rights. We spoke to both of the people who lived at Compass Grove who were positive about the care they experienced one person told us, 'they look after me very well, I like it here, I don't want to live anywhere else'. Another person said, 'I like Bernadette very much she is a great person, if I ever have a problem she helps me'.

People who use the service, staff and visitors were protected against the risks of unsafe or unsuitable premises. We toured the building and found that it was light and airy. All of the rooms in the house including the bedrooms were clean and well maintained.

The location had enough qualified, skilled and experienced staff to meet people's needs. However, its staff rota planning could be improved.

People were not protected from the risks of unsafe or inappropriate care and treatment because accurate and appropriate records were not maintained. Although staff we spoke to were aware of people's needs there is a possibility information about a person may be overlooked.

During a routine inspection

People told us that they were very happy at Compass Grove. Both of them had lived at the home for more than ten years. They showed us their rooms that were highly personalised and they told us how they were involved in choosing the different furniture and the decoration. One of them told us 'I'll never move away from here' and that 'We've got a lovely staff'. People told us that they've got a busy social life and they were supported to work in the community.

We found that people's care was reviewed regularly and their needs were met. People were involved in various activities in the home and in the community. One person worked four days a week in a charity shop and was involved in fundraising regularly and the other one was about to start to work in a new place.

A senior member of staff told us that 'I love working here I think it's fantastic' as staff were able to spend quality time with people and build a supportive relationship with them.

During an inspection looking at part of the service

We did not visit the service or speak with people who use the service on this occasion. We visited this service in November 2011 and found that people who used this service could not be confident that the service was robustly monitored by the provider to ensure that it was safe and met their needs. The provider subsequently confirmed that they had engaged the services of a consultant to carry out monitoring visits at the service and that this started in May 2012. Therefore we were satisfied that appropriate monitoring systems were in place.

During a routine inspection

People who use the service and their relatives were very happy with the care and support provided. People who use the service said: 'It's lovely living here, I've been here a long time and I don't want to leave here. I'm happy. The staff are nice and we've got a nice pet. The staff treat me good and I like my job.' 'Yes I like living here, I've been here for years. It's a nice house it's a normal size, not too big and not too small. The staff are nice.' Relatives said: 'They have done a brilliant job with my daughter. She has developed a lot over the last few years.' 'I have no problems with the service whatsoever. I am happy with it and my daughter is very happy there. She has come on in leaps and bounds. She can express herself and would say if she did not like anything. She is very settled there. She is very busy, goes to clubs and on holiday. She is very safe there and I have no doubts about that. She is very much at home.'

Staff told us: 'They do have residents meetings and people say what they want. They are very active, they go to clubs, keep fit, pantomimes, shows and on trips. They are involved in their care plans and know what's in them. They get a lot of say in what they do and are treated properly.' 'They do most things around the house. They make drinks, meals, do their rooms and their washing.'

A healthcare professional said, 'My client is always busy. The manager is very good. If there are any problems she communicates them promptly. My client is stable at present. She is encouraged to do a lot for herself and to maintain her skills and it's fantastic that she is doing the hairdressing course.'
