Well-led
The provider had made some improvements to put right the shortfalls and regulatory breach we found at our inspection on 18 March 2026. We have told the provider to take action. We will be following up on our concerns to ensure the provider has made the required improvements.
During our inspection of this key question, we found
the registered person had ineffective systems or processes to enable them to assess, monitor and improve the quality and safety of the services being provided. We also found ineffective systems or processes to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk.
This resulted in a breach of Regulation 17, Good governance.

Governance, management and sustainability
The provider’s systems for governance and oversight of the service were ineffective and their implementation at the service was not monitored effectively.
Audits of infection prevention and control (IPC) were now carried out. We noted that these audits did not reflect our findings during this and previous inspections and that the template used no longer reflected current guidance.
Monitoring of recruitment processes were still not effective in ensuring all required pre-employment information was available for all staff. Records to confirm that all staff had obtained a Disclosure and Barring Service (DBS) check in this employment were not available.
These issues were identified at the previous 2 inspections.
Systems to monitor the completion of recommended training and continuous professional development (CPD), were not established. The provider could not demonstrate effective oversight of completion of CPD for all staff.
The provider had made the following improvements.
Radiography audits were now carried out in line with recognised guidance and action taken to address any issues when required.
