Well-led
The provider had made insufficient improvements to put right the shortfalls and had not responded to the regulatory breach we found at our inspection on 10 September 2025. We have told the provider to take action. We will be following up on our concerns to ensure the provider has made the required improvements.
During our inspection of this key question, we found
the registered person had ineffective systems or processes to enable them to assess, monitor and improve the quality and safety of the services being provided. We also found ineffective systems or processes to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk.
This resulted in a breach of Regulation 17, Good governance.
You can find more details of our concerns in the detailed findings below.

Governance, management and sustainability
The provider’s systems for governance and oversight of the service were ineffective and their implementation at the service was not monitored effectively.

Staff could not consistently demonstrate the process and equipment used to carry out these checks.
Processes were not established to ensure required monitoring checks to reduce the risk and spread of Legionella were carried out in line with guidance. Staff could not consistently demonstrate the process and equipment used to carry out these checks.
Required audits of infection prevention and control and radiography were not carried out in line with guidance. We were only provided with one audit of infection prevention and control; this did not reflect our findings at this or our previous inspection.
