Assessing needs

Score: 3

Staff told us there were effective systems in place to assess and monitor people’s needs. This included handover meetings between incoming and outgoing staff on shift, staff meetings or memos. Senior staff were responsive to feedback on day 1 of our inspection around improving people’s dining experience. By day 2 of the inspection, people’s care plans had been updated and we observed improvements around meeting people’s eating and drinking needs.

People and their relatives told us their care needs had been assessed thoroughly and they were involved in care planning. Comments included, “From the very beginning of planning for my parents to move into Southampton Manor I have been involved in all aspects of care. I always feel that I can raise any concerns that I may have with a senior member of staff who are receptive and supportive to any matters that I raise however small” and, “Yes I have been involved in [developing] my care plan. It reflects me."

Staff used a range of assessment tools to assess and monitor their care needs, nursing needs, health and medical conditions and wellbeing. Initial assessments of people’s needs were carried out when people were first admitted to the service and there were systems in place to review assessments at regular intervals or when people’s needs changed.
