During an inspection in response to concerns

We carried out this inspection because we had received information that the service was not meeting people's needs. We reviewed the care records of people using the service and found that people's needs were assessed and care and treatment was planned and delivered in line with their individual care plan. However the manager informed us that sometimes there was a lack of communication between services as they had not been informed in advance by other professionals when they needed to provide a service to someone in the community. The manager told us that they were working hard at improving communication between services.

People we spoke with told us that they were happy with the care that they receive. One person told us 'the staff are very good and I am 100% happy with the service".

People's needs were assessed and care and support was planned and delivered in line with their individual care plan. We looked at the care records of people and found them to be comprehensive. They provided staff with all of the information that were needed to support people.

People told us they were given opportunities to express their choices and to make decisions in their daily lives. Staff were aware of people's preferences and routines so they could support them. For example they knew at what time people preferred to go to bed or get up and what assistance they needed. People were supported in promoting their independence.

It was clear from the care plans and records we saw that where possible, people who used the service had been involved in them.

During a routine inspection

People who used the service understood the care and treatment choices available to them. People we spoke with told us they were treated with respect. One person said "she (the carer) is very friendly" and a relative told us "the carer is very polite." Staff we spoke with told us how they promoted people's privacy, choice and independence. Most people we spoke with told us they were satisfied with the care and support provided by the service. One person said "they know what my needs are. They are brilliant." We found care plans were in place, but in some areas these contained only limited information on how to support people.

We found that staff had access to on-going training, including training about health and safety, moving and handling and safeguarding vulnerable adults. Staff were able to locate records promptly, although we found that not all confidential records were stored securely. The service had systems in place for monitoring the quality of care and support provided.

During a routine inspection

The service focuses largely on the rehabilitation needs of people being discharged from hospital by providing support to rehabilitate them back in to the community. People told us that they were treated with respect and were included in decisions about their care and treatment. One person said "I received the care that I expected. I am now able to do a lot for myself." Another said "they were always respectful and provided a very good service, they were not intrusive." People who used the service and their relatives were happy with the quality of care they received.

People said that the reablement carers responded to their needs appropriately and that they had the right skills to support them. We found that the reablement carers had sufficient skills and a good understanding of people's individual needs. People said they felt safe with the carers. There were systems in place to assess and monitor the quality of service provided.
