Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.

People told us the care and support provided was good and personalised to their needs.Staff supported people on visits to places they were interested in and encouraged people to take part in social activities.

Staff said “I always make sure I read their care plan and see what we are doing, read a little about them, see what they like. If someone has been before me I will get in touch with them to get their tips, thoughts and things to help me”

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. The provider had developed good links with community organisations and agencies.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. People’s individual communication systems and styles were respected and valued. Information was available in a variety of formats and could be tailored to peoples’ individual needs.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result.

People told us they could always contact managers when they wanted to. They told us the management team were approachable. Relatives told us they felt involved and were kept informed about their family member’s care. They told us they felt comfortable with the staff and that any concerns or requests were listened to by the carers. One said, “The management is approachable, but there is no need to call to the office.”

Equity in access

The provider made sure people could access the care, support and treatment they needed when they needed it. Care plans were tailored to reflect individuals’ cultural, religious, and personal preferences, and staff received training to support inclusive and respectful care. People told us they felt understood and valued, and adjustments were made to ensure their care was delivered in a way that suited their lifestyle and communication needs.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. They used this to make changes that helped people feel more included and supported. This included adapting care for those with different communication needs or health conditions. People told us they felt their care was shaped around them and that staff understood what mattered most to them.

Planning for the future

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life. People were able to share their end of life wishes if they wished to.
