Condition of this registration relating to carrying out this regulated activity

a. The registered provider must implement and maintain a continuous quality improvement system that
identifies areas for improvement through audits, feedback, incident reviews and performance data.
b. The registered provider must maintain a service improvement plan that reflects condition 3 and includes:
(i) Clear actions to address each
identified issue
(ii) Timescales for completion of each
action
(iii) The individual responsible for each
action with documented evidence of
their competency
(iv) A risk assessment for each action
with the documented controls to mitigate
risks during implementation.
c. The improvement plan must include but is not limited to, fire safety and evacuation, medication, dependency levels of service user, staffing levels, skills and competency, leadership
oversight including quality assurance.
d. The quality improvement policy and quality improvement plan must be submitted to the Commission on the last Friday of every month. Each submission must include updates on progress, amendments and outstanding actions.

Terms of this registration relating to carrying out this regulated activity

The registered provider must not provide nursing care under accommodation for persons who require nursing or personal care at Herondale.

The registered provider must only accommodate a maximum of 8 service users at Herondale.
