Our view of the service

Vine house older persons residence is a 'care home' that provides accommodation for older people, some of whom require support with their personal care needs.

This home is not required by condition of registration to employ a manager registered with CQC. However, the provider chooses to do so. There had been 4 managers in the past 3 years, which had caused some disruption in the governance of the home. The provider has appointed another manager and is awaiting checks and a start date.

The home was not always consistently and effectively managed by leaders. Changes in management, revised management structure and governance responsibilities, system changes and inconsistent delivery of quality assurance processes had meant that leaders did not always have consistent and organised oversight of the home.

Improvements were needed on the home’s approach to assessing people’s capacity while more work needed to be completed to ensure that staff had the skills and readiness to use systems effectively and maintain consistency.

Whilst there were improvements needed to governance structures, care systems, staffing and quality assurance systems, we found that this did not impact on the care being provided. Staff treated people as individuals. People were encouraged to maintain relationships with family and friends. A relative told us, “Visiting the home is very simple and the staff are always welcoming and offer a drink when I visit. I like the fact that it is relatively small which enables Vine house to have a friendly home from home vibe.”

Staff responded to people in a timely way. People received personalised care that was tailored to meet their individual needs, preferences and choices. Care plans were being updated where needed and guided staff about people's needs and how to meet them. Concerns and complaints were listened to and used to improve the home they received.

Staff provided safe, effective and responsive support. Systems and processes were in place to mitigate the risk of harm to people and to protect them from abuse. Staff understood their responsibilities in reporting potential safeguarding issues, while the provider understood their regulatory responsibilities. People were involved in the management of risk to their care and support. People were supported by enough staff. Appropriate infection control procedures were in place, and the environment was kept clean and a safe place for people to live.

People were involved in assessments of their needs which took into account people’s communication, personal, health and any equality needs. Staff made sure people understood their care and support needs which enabled them to give informed consent.

 

People's experience of this service

People told us that they were happy with the home and spoke positively about the care and support provided. People said they were happy living there and described the home as open, friendly and welcoming. People told us they enjoyed the activities which took place and had particularly enjoyed a recent garden party where family, friends and local people had been invited.

People said staff always treated them with respect and kindness, and made sure any personal care was provided sensitively, preserving their privacy and dignity. We carried out a number of observations throughout the inspection and staff interacted with people sensitively, safely and compassionately. One person said, “Staff are gorgeous, friendly, helpful good group of people.”

People were involved in assessments of their needs. Staff reviewed assessments taking account of people’s communication, personal and health needs. One person told us about their choices and how staff knew them well, “They always ask me what I want in terms of food each day and they give me a choice from a menu and one day I thought I'll be cheeky and I'm going to put I want James Bond for breakfast, the next day my breakfast came up and James Bond wasn't there, then when the boss came over to have a chat he asked me if everything was alright like he normally does and I said no I have a complaint to make I asked for James Bond to be at breakfast but he wasn't there, then guess what happened the next day well my breakfast came up and there he was a picture of James Bond it was Sean Connery, my favourite, and if you look behind you there is up on the wall in a frame the maintenance man helped me put him up there now they didn't have to do that that's just a little example of the extra mile they go.”
