During an assessment under our new approach
Date of Assessment: 14 April 2025 to 23 April 2025. Upper Thames Medical – The Park Surgery and Lechlade Medical is a GP practice which delivers services to 11064 patients. The practice offers services from the main practice and the branch surgery in Lechlade under a contract held with NHS England. Information published by the Office for Health Improvement and Disparities shows that deprivation within the practice population group is in the 9 decile (9 of 10). The lower the decile, the more deprived the practice population is relative to others. This assessment considered the demographics of the people using the service, the context the service was working within and how this impacted service delivery. We assessed all quality statements across the safe, effective, caring, responsive and well led key questions. We found staff took concerns seriously. When things went wrong, staff acted to ensure people remained safe. Staff were caring and understood the importance of providing an inclusive approach to care and made adjustments to support equity in people’s experience and outcomes. There are clear responsibilities, roles, and systems of accountability. The practice reviewed and responded to feedback and continued to improve and embed new systems and processes. Our remote clinical searches found gaps in medicines monitoring; however, the service addressed these immediately and implemented new processes where necessary.
