During a routine inspection

Prior to our inspection we reviewed all the information we had received from the provider. We spoke with four people who used the service to establish their views on the quality of service provision. We also spoke with the registered manager, a qualified nurse and two care staff. We looked at some of the records held in the service including the care files. We observed the support people who used the service received from the care staff and carried out a tour of the building.

We found that staff obtained peoples consent before interventions were performed and when people lacked capacity to provide informed consent, the provider had acted in accordance with legal requirements.

We found the provider had ensured that people benefited from a choice of suitable and nutritious food and drink in sufficient quantities to meet their individual needs and preferences. Comments included, 'The food is very nice and there's always a choice,' and, 'I cannot grumble about the food at all. I like to have small portions and the staff know that and respect my wishes.'

We found that staff were only employed once an effective recruitment and selection process had been undertaken. People told us they felt the staff had the right qualifications, skills and knowledge to perform their duties. One person told us, 'All the staff are very good at what they do'. Whilst another said, 'These ladies will do anything for us. All the staff are tremendous, absolutely fabulous.'

We asked people if they felt safe and comfortable living at the service and if they were satisfied with the quality of the premises. One person told us, It's lovely and comfortable and always very clean,' whilst another said, 'I cannot grumble at all, I feel safe here and it's always clean and fresh.'

We found there were safe and accessible communal spaces within the home to promote privacy and choice. We also found that the premises were maintained to a high standard of decor and cleanliness.

We found that systems were in place to enable people to complain or make comments about the quality of the service they received. People also told us they felt confident in reporting any concerns or complaints and felt safe whilst residing at the home.

During a routine inspection

Due to the complex needs of some of the people who lived at the service we used a number of different methods to help us understand their experiences. We looked at records. These included care records and information about how the service operated.

We spoke with eight people, two relatives, a visiting social worker two staff members, the registered manager and the home owners.

One person told us, 'I wouldn't change a thing about living here. It is my home and I feel like I have what I need.' Another person said, "I know there is always a staff member to talk to if I need any help." A visiting relative commented, 'They (staff) look after people really well in our opinion. We feel our family member is safe and well cared for.'

We also spent some time in one of the communal areas of the home. We observed how staff used their training and experience to interact with, and care for people who had needs associated with dementia.

We found staff communicated with people by speaking with them directly, and through the use of non verbal actions. We saw this helped to ensure people were able to express their feelings and views and wherever possible were involved with any decisions that needed to be made about their care.

People were supported sensitively and their dignity maintained when staff provided physical care. Staff also encouraged people to take part in meaningful activities.

3 October 2011

During a routine inspection

We visited the service on 10 October 2011. We spoke with 4 people who told us that they were happy living at Foxby Hill. One person told us that they had lived there for several years and would never want to move. They said the care was right for them and they were able to choose everything from the food they ate, to how they were supported by staff.

People told us that they had opportunities to go out with the activities co-ordinator and there were always activities to take part in if they wanted to. All the people we spoke with told us there were always plenty of staff on duty who took time to talk with them if they were worried about anything.

One person told us that the staff were very good, kind and caring and were always there when needed.

We observed that people seemed very relaxed and content and this was confirmed by those we spoke with.

A relative told us that they were always made welcome and staff kept them informed about their relative. 'The staff always seem to be able to tell me how my relative has been. They all know him.' 'I feel involved in my relatives care.'
