Date of Assessment: 9 March 2026.

Altogether Care – Care at home Limited Salisbury is a homecare agency registered to provide support to people living in their own homes. The service supports people living with dementia, people with learning disabilities, physical disabilities and mental health conditions. At the time of our assessment there were 100 people receiving a regulated service. The inspection was carried out because of the length of time since the last assessment.

We assessed the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed autistic people and people with a learning disability respect, equality, dignity, choices, independence and good access to local communities that most people take for granted.

The assessment was a fully comprehensive inspection, which looked at all key areas.

We identified some shortfalls with the provider’s governance systems. We identified 1 breach of the legal regulations relating to good governance. We have asked the service to provide us with an action plan to show how they will meet this breach.

Care being delivered on time and inconsistent visits were a concern. This was not identified in the provider’s audits. The provider told us they would look at timing of visits and develop a plan to address this.

Staff had been recruited safely with all pre-employment checks completed prior to them starting. Staff were not receiving supervisions in line with the provider’s policy. People’s risks had been mitigated and well documented. Staff told us leaders and managers supported them well. All staff had completed safeguarding training and expressed confidence any concerns raised would be addressed appropriately by leaders. However, there were some shortfalls with consent records which lacked documentation relating to people’s capacity. Most care plans contained personal preferences regarding how people liked their care to be delivered.

Staff protected people’s privacy and dignity. They treated people as individuals and supported their preferences. People had choice in their care and were encouraged to maintain relationships with family and friends. However, some staff told us they were expected to work longer hours and every weekend while others had every other weekend off.

Environmental safety was embedded into everyday practice. Staff were trained to identify hazards and escalate concerns promptly, supported by regular competency checks and scenario-based learning. Proactive referrals ensured people’s homes were adapted to meet changing needs. This enabled people to feel safe and respected in their own environment.

People's experience of the service

We spoke to 12 people who received a service. We received mixed feedback from people and their relatives. People told us they were happy and felt safe. Comments from people included “The care is fantastic, the staff are fantastic, they are kind to me” and “They do everything I need them to do”. Another person told us, “I get good carers everyday”.

People praised the provider for working together with other stakeholders, commenting “They have suggested some improvements to district nurses and GPs, which have been really useful” and “Training seems fine, on a couple of occasions I think they’ve worried too much but better that than not caring”.

However, people raised concerns about the timings of their visits. Comments included “They know I go to bed early, around 8:00pm and the carers have regularly arrived at 9:30pm” and “My visits are scheduled for 8:00am, but staff invariably turn up at 9:30am.” One person told us “My regular carers aren’t usually late, but new staff are”. Several people told us their staffing preferences were not always listened to. Comments included: “Office staff have been sympathetic when I have called regarding visit timing, but nothing has really changed”. However, several people told us “I asked for female only carers and Altogether Care has always respected that”.
