The practice had a governance system that included policies and procedures.

 

Critical examinations of x-ray set in surgery 4 and 5 and highlighted that the x-ray beam was directed at a partition wall. Evidence was available to confirm this had been assessed and any potential risk mitigated.

 

The fire risk assessment was carried out by someone who could demonstrate competency in the management of fire safety.
 

General waste bins were stored away from the fire escape route. The bins were locked to prevent unauthorised interference.

 

Unused boxes of personal protective equipment that were present under the fire escape route external steps had been removed.

An annual infection prevention and control statement was available.

Evidence of oversight of surgery cleaning standard was available.

Clinical staff could tell us who the infection control lead was.

 

Mops used to clean clinical areas of the practice were stored outside the building which meant that extreme cold/wet weather conditions could hinder effective storage arrangements. We have since received evidence to confirm this shortfall has been addressed.

 

The practice had evidence to confirm that external laboratories they used were registered with the Medicines and Healthcare products Regulatory Agency (MHRA).

 

A hearing loop was available.

Vision aids (magnifying glass/reading glasses) were available.

Language translation services were available.

The access needs of hearing and sight impaired patients were considered as part of a disability access audit that was carried out in October 2024.

 

Radiography audits were carried out in line with national guidance (100 radiographs every six months). Radiography audits were carried out in line with national guidance. However, improvement should be made to include all of the relevant information to enable an effective review. We have since received evidence to confirm this shortfall has been addressed.

 

Infection prevention and control audits were carried out six monthly in line with national guidance.

 

General data protection regulation (GDPR) accident books were in use.

Completed accident records were removed and stored securely.

 

The patient complaints procedure in the waiting room enabled patients to access the document independently.

Records relating to complaints were stored securely.

 

We noted 2 shortfalls that remained outstanding:

A critical examination was not available for x-ray machine in surgery 1. We were told that the practice was waiting for the document to be received from the engineer who installed the machine.

NHS prescription pad stock controls and security arrangements in treatment rooms was not effective.

We have since received evidence to confirm these have all now been addressed.
