Date of Assessment: 11 December to 19 December 2025. York Street Health Practice is located at 68 York Street, Leeds, West Yorkshire, LS9 8AA. The practice is located on the outskirts of Leeds City Centre in a building which operates across 3 floors, with York Street Health Practice occupying the ground and 2nd floors.
York Street Health Practice is a specialist inclusion health GP service which aims to address health inequalities faced by Inclusion Health populations, including people experiencing homelessness, refugees and people seeking asylum, Gypsy, Roma and Traveller communities and sex workers across West and North Yorkshire.
Services are delivered under an Alternative Provider Medical Services (APMS) contract held with NHS West Yorkshire Integrated Care Board (ICB).
Due to the patient demographic, there is a high turnover to the registered patient list. At the time of our inspection the confirmed registered list size was 3,074. We were advised that 71.4% of these patients had registered in the last 12 months. The practice serves a population who are predominantly male (85%), with 61% of registered patients being aged between 21 and 40 years.
Information published by Office for Health Improvement and Disparities shows that deprivation within the practice population group is in the first decile (1 of 10). The lower the decile, the more deprived the practice population is relative to others. According to the latest available data, the ethnic make-up of the practice area is 68% White and 15% Asian with the remaining 15% being Black, Mixed and other non-white backgrounds. This assessment considered the demographics of the people using the service, the context the practice was working within and how this impacted service delivery. The provider is registered with the Care Quality Commission (CQC) to deliver the Regulated Activities: diagnostic and screening procedures; family planning; maternity and midwifery services and treatment of disease, disorder or injury.
An inspection had previously been carried out in September 2018, when the service was provided by a different legal entity. We carried out this announced, comprehensive assessment as the provider of the service had changed. At this assessment we found:
